RESUMO:

Optimizing waiting time for hospitalization has been a major problem for the Unified Health System (SUS), and for the quality of health services. Delay in care has significant impacts on well-being, chances of cure, the nature and extent of sequelae in the patients and families involved. The situation is even more aggravating when, besides being high, the deadlines are unpredictable. According to Ordinance No. 1,559 of August 1, 2008, which establishes the National Policy for SUS Regulation, implemented in all federated units, respecting the competences of the three management spheres, as an instrument that enables the fullness of health responsibilities, assumed by the spheres of government: Health Systems Regulation; Health Care Regulation; and Regulation of Access to Assistance, this entire management process should be guided by the profile of institutional complexity, defined according to the agreed model, following pre-established criteria between Institutional Management and State, Municipal or Federal management. Contractualization is a process of instrumentalization with qualitative and quantitative goals, created by the Ministry of Health to ensure care financing. Adapting this reality to a university hospital is essential to equalize demand and productive capacity. To be functional, it is necessary to create strategies for stratification of the waiting list according to criteria that allow the call of patients to occur in an equitable, effective and free of personal interests. The purpose of this chapter is to show that with the creation of an intelligent inpatient waiting list management system, it is possible to optimize the waiting time of patients for clinical hospitalization in the different specialties and, consequently, reduce morbidity and mortality, increasing the occupancy rate. and bed turnover, minimizing the hospital stay rate. Several benefits and advantages have been reported in the Inpatient Management (or waiting list management) process for patients. On the other hand, although efficient bed management has its advantages, the lack of articulation between theory and practice, issues related to ethical demands, limitations to the development of essential competences, strategy to develop tools for agility, Legal recommendations and the current structure of hospital units for this management are factors that often hinder the continuity of the process.
