Introduction: Supervision of vaccination rooms is essential to ensure the quality of care in immunization services. To this end, tools in the form of checklists or printed forms are used. Despite technological advances, these tools continue to be applied manually, often with limitations related to the standardization and systematization of data collection, lacking standardized criteria for qualifying the results obtained. Aim: To design and develop a computerized tool in the form of a web application based on scientific evidence, capable of optimizing the supervision process, supporting decision-making and strengthening the quality of immunization services in the context of Primary Health Care. Methodology: This is a scoping review following the JBI guidelines. To design and develop the computerized instrument in the web application format in this study, we considered the hybridization of evidence from the scoping review and items from the printed instrument used by the State Department of Public Health of Rio Grande do Norte (adapted from the Vaccination Room Supervision Instrument Assessment Program, version 2.0, December 2004). In addition, Brazilian legal guidelines were consulted, such as the Manual of Standards and Procedures for Vaccination (2024) and the Cold Chain Manual of the National Immunization Program (2017). Results: Of the 3,506 studies identified, 32 were included in the final sample. Among the main potentialities presented in the literature, the organization and optimization of work processes, the performance and engagement of the nursing team in decision-making, and the strengthening of the self-efficacy and confidence of health professionals stand out. Barriers such as operational and organizational failures, overload of functions and activities, and the underuse of data from the immunization information system were highlighted. The prototype of the computerized tool in web application format has 13 screens (login, identification, technical team, general aspects of the room, technical procedures, cold chain, ESAVI, special immunological agents, health education, information system, instruments present in the room, manuals present in the room and doses found). In this prototype, a qualification of the results adapted from the Forrester Vaccination Room Adequacy Index will be adopted. Conclusion: The main supervision instrument identified was the PAISSV of the Ministry of Health (Brazil). Other instruments and adaptations considered relevant for the proposal of the computerized instrument were identified, in addition to the Brazilian legal guidelines. Thus, the design and development of a computerized instrument with the potential to optimize the supervision work process in vaccination rooms and improve the quality of the health service was encouraged
