RESUMO:

Objective: The purpose of this investigation was to evaluate the eosinophil and eosinophil cationic protein (ECP) in urine of SLE patient, either with or without LN, as a possible laboratory marker to predict future renal disease activity of SLE patient. Methods: Seventy-four patients with SLE, of which 20 with clinical and laboratory evidence lupus nephritis (LN group) and 54 without evidence renal disease involvement (non-LN group) were analyzed regarding eosinophils in urine and serum and urinary ECP. The eosinophiluria was observed by Hansel's stain and ECP by fluoroenzymeimmunoassay. Statistical analysis was performed using Spearman correlation, Student's t-tests and Mann-Whitney U test, multivariate linear regression and operating characteristics curves. Results: Statistically significant differences (p<0,001) were observed between LN group versus non-LN group in variables urinary eosinophils number, serum and urinary ECP and ECP-Cr. The correlation was positive and statistically significant (p<0,001) between urinary ECP with urinary eosinophils number (r_s= 0.70), ECP-Cr ratio (r_s= 0.63) and the ECP-Cr ratio with MEX-SLEDAI (r_s= 0.72), Pr-Cr ratio and proteinuria 24 h (r_s= 0.70), serum creatinina (r_s= 0.70), haematuria (r_s= 0.76) and anti-dsDNA (r_s= 0.51). There was also a significant association between urinary ECP and eosinophiluria and between ECP-Cr ratio and urinary ECP, Pr-Cr, creatinine and Mex-SLEDAI. ROC analyses showed ECP-Cr ratio better AUC as eosinophiluria parameters and renal inflammation in SLE parameters (0.79 and 0.92) respectively.Conclusions: In conclusion, that among the parameters associated with eosinophiluria the urine ECP-Cr ratio is good marker of renal inflammation in SLE patients.
