SUMMARY:

Objective: To investigate longitudinal changes of 24h movement behaviors (24hMB) and their correlates in older adults.

Methods: The 24hMB was assessed in older adults aged 60-80 years using hip accelerometry over one week at baseline and after four years. Outcomes included sleep duration, sedentary time (ST), light physical activity (LPA), moderate-to-vigorous physical activity (MVPA), step count (total; incidental: <40 steps/min; purposeful: 40+ steps/min), and peak cadences (1-, 30-, and 60-min steps/min). Associations between changes in 24hMB and correlates (age, sex, body mass index, post-secondary education, and multimorbidity) were analyzed using generalized estimating equations.

Results: A total of 86 participants were analyzed (67.4±5.3 years). No changes were observed in sleep (p>0.05). However, ST increased (+35 min/day), while LPA (-16 min/day), MVPA (-9 min/day), step count (total: -1139 steps/day; incidental: -122 steps/min; purposeful: -1017 steps/min), and peak cadences (-8 steps/min) reduced over four years (p<0.05). Older age was associated with reduced sleep duration, increased ST, and fewer total and incidental steps over time. Female sex was associated to reduced sleep duration, increased ST, higher MVPA, more purposeful steps, and higher 30- and 60-minute peak cadences. Higher BMI was associated with reduced purposeful step count and 30- and 60-minute peak cadences, while post-secondary education was associated with increased total step count and higher 30- and 60-minute peak cadences (p<0.05). No associations were found between multimorbidity and changes in 24hMB (p>0.05).

Conclusion: After a 4-year period, older adults experienced an overall increase in ST and a decrease in PA, while sleep remained stable. We identified harmful and beneficial correlates of changes in sleep, ST, and PA, which could inform targeted interventions to improve 24hMB in this population.
