SUMMARY:

INTRODUCTION: Wounds represent a major public health challenge due to their high prevalence, their impact on patients's quality of life, and the substantial costs associated with treatment. In this context, natural products with bioactive properties, such as stingless bee honey (Meliponini), have attracted increasing attention because of their therapeutic potential in wound healing. Although experimental and clinical studies have demonstrated promising results, the available evidence remains scattered throughout the literature, justifying the need for this systematic review. OBJECTIVES: This study aimed to analyze the effectiveness of stingless bee honey in wound healing by identifying the most extensively studied stingless bee species, the formulations used, and the outcomes reported in different experimental and clinical models. METHODOLOGY: A systematic review with meta-analysis was conducted following the PRISMA 2020 guidelines and registered in PROSPERO under protocol CRD420251001075. Literature searches were performed in PubMed/MEDLINE, Scopus, Embase, Web of Science, the Cochrane Library, LILACS, SciELO, CINAHL, and ScienceDirect, in addition to gray literature sources. Clinical, in vivo, and in vitro studies evaluating the use of stingless bee honey or its derivatives for wound treatment were included. Study selection was independently performed by two reviewers, and methodological quality was assessed using design-specific risk-of-bias assessment tools. RESULTS: The findings demonstrated that stingless bee products promote tissue repair through multifactorial and synergistic mechanisms. These products exhibited antimicrobial activity against common wound pathogens, including multidrug-resistant strains such as methicillin-resistant Staphylococcus aureus (MRSA), Staphylococcus aureus, and Pseudomonas aeruginosa, and were also effective in disrupting biofilms. They showed high antioxidant capacity, particularly through free radical scavenging (DPPH assay), protecting cells from oxidative stress that may impair wound healing. Furthermore, they modulated the inflammatory response by regulating cytokines, including TNF-α, IL-1β, and IL-6, while stimulating growth factors such as vascular endothelial growth factor (VEGF). These mechanisms promoted accelerated wound contraction, enhanced migration and proliferation
of fibroblasts and keratinocytes, and improved collagen deposition and tissue organization. In humans, the randomized clinical trial conducted by Rosli et al. (2024) demonstrated that Kelulut honey showed efficacy comparable to conventional hydrogels in the treatment of diabetic wounds, promoting granulation tissue formation and re-epithelialization. However, methodological heterogeneity among the included studies and the limited number of clinical trials restrict the generalizability of the findings. CONCLUSION: Stingless bee honey and other stingless bee-derived products exhibit promising therapeutic potential for wound treatment by acting through the integration of antimicrobial, antioxidant, immunomodulatory, and wound-healing mechanisms. Nevertheless, well-designed and methodologically standardized clinical trials are still needed to strengthen the current evidence and support the incorporation of these products into clinical practice.
