SUMMARY:

Cardiovascular diseases stand as the leading cause of mortality in Brazil and worldwide, contributing significantly to hospitalizations and placing a substantial burden on public services. Crucial and effective interventions for the prevention and control of these illnesses involve the stratification of cardiovascular risk and the management of modifiable risk factors in the population. This research aims to identify and analyze cardiometabolic risk factors in a prospective cohort. The study adopts a prospective cohort design and is linked to the multicentric study "Estudo Brazuca: Brazilian Usual Consumption Assessment, CAEE n° 60777522.0.0000.5292." A comparative analysis was conducted between data collected from the initial cohort of 112 participants in 2019 and the current data in 2024, following the same methodological criteria as the initial analysis. Collected data encompassed sociodemographic, clinical, biochemical, and anthropometric parameters. The global risk score was calculated, and cardiovascular risk was defined. Anthropometric parameters (BMI, waist circumference, and hip circumference) and biochemical parameters (triglycerides, total cholesterol, LDL, and HDL) were evaluated. Additional indices were computed for cardiometabolic risk stratification. Parametric testing was employed for variable variation comparison. The ROC curve was utilized to identify the cardiovascular risk index with the best predictive capacity. A correlation test was conducted between the best predictive index and anthropometric and biochemical variables. An increase in the variation of all analyzed parameters was observed during the pandemic, with noteworthy changes in triglycerides (4.55 mg/dL, p=0.680), lowdensity lipoprotein (3.93 mg/dL, p=0.503), and waist circumference (3.36 cm, p=0.487). The Plasma Atherogenic Index (PAI) demonstrated a significant predictive value compared to the gold standard (AUC=0.21, p=0.031) and exhibited a positive and clear correlation with Body Mass Index (BMI) (p=0.047, rho=0.372). In conclusion, there was an increase in biochemical and anthropometric parameters during the pandemic, potentially contributing to the risk of cardiovascular diseases. Finally, the control of these modifiable risk factors and the use of the PAI can serve as adjuncts to primary prevention and risk stratification for cardiometabolic diseases, respectively, assisting in the prevention of illnesses and health complications in the studied population.
