SUMMARY:

The consumption of ultra-processed foods (UPF) has increased exponentially in the population's diet, having a nutritional composition with characteristics of high energy density, free sugar, total fats, saturated fats and trans fats. Although a greater proportion of UPF in the diet is associated with greater adiposity and the risk of developing excess weight at different stages of life, its impact has not yet been studied during lactation, a stage in which the quality of the diet is important for reducing weight loss, postpartum weight retention and obesity prevention in adult women. Therefore, the objective of this study was to evaluate the association between UPF consumption and the anthropometric profile of lactating women assisted in primary health care in Natal-RN. Socioeconomic and health data, maternal food consumption through 24-hour recall and anthropometric measurements of lactating women between 30 and 150 days postpartum were collected. Food consumption was assessed using the NOVA Classification and the anthropometric profile according to pre-gestational Body Mass Index (BMI), gestational weight gain, postpartum weight retention (kg), presence of excess weight according to current BMI and measures of body perimeters (cm) and skinfolds (mm). Participants were grouped according to tertile of energy participation of UPF in the diet (tertile 1-2 x tertile 3). Crude and adjusted linear regression models were analyzed seeking to evaluate the association between greater participation of UPF (exposure variable) and measures of the anthropometric profile of breastfeeding women (outcome variables). The women were on average 28 (7) years old, 74 (46) days postpartum, 34.7% had low education, the majority had low income (82.3%, n=102) and the participation of caloric value of UPF in the diet was 23.31% (0- 76%). Excess weight was present in 65.5% of them and 40.9% of them had retained more than 4 kg in the postpartum period assessed, with an average retention of 2.40 kg (95% CI 1.24- 3.56). After adjusted linear regression analysis, a positive association was found between the highest percentage of UPF consumption and postpartum weight retention (β= 4.54, 95% CI 2.07-7.01, p< 0.001) and the measurement of hip perimeter (β= 6.51 95% CI 0.56-12.45, p = 0.032). The results showed that a greater participation of UPF in the diet of lactating women was related to anthropometric measurements associated with adiposity, which reinforces the need for actions aimed at reducing the consumption of UPF during lactation.
