SUMMARY:

Non-alcoholic fatty liver disease (NAFLD) has several stages, from lipid deposition in the liver parenchyma to cases of hepatitis, fibrosis, cirrhosis and hepatocellular carcinoma. In situations of liver injury, there is an increase in the concentrations of hepatic transaminases, especially alanine aminotransferase (ALT). Inadequate food intake, with emphasis on excessive food intake in free sugars, has been considered one of the causal factors for the development and progression of this disease. In this context, currently, fructose, especially that from unnatural sources, has been studied as an independent causal factor in the development of NAFLD and unfavorable metabolic outcomes. Thus, the aim of this study is to investigate the relationship between fructose consumption by degree of food processing with liver markers and metabolic parameters in overweight adolescents. This is a cross-sectional study with adolescents aged 10 to 19 years, diagnosed with overweight and obesity. Anthropometric data (height, weight and waist circumference), biochemical data (lipid profile, glucose and ALT concentrations) were collected, and two 24-hour recalls were applied to analyze food consumption. To quantify the fructose consumed, the foods present in the recall were matched with those in the Food Standards Australia New Zealand. Soon after, the foods were classified according to the food groups of the NOVA classification, by their degree of processing. With this, the amount of fructose from each food group was calculated. In the data analysis, the procedures of descriptive statistics of mean and standard deviation and confidence interval of the mean were used for the quantitative variables and frequency distribution and their 95% confidence intervals (95%CI) for the categorical variables. In order to identify possible differences in the adolescents' profile, the chi-square test was applied for categorical variables and Student's “t” test or ANOVA to compare means. To verify the association between fructose consumption and nutritional and metabolic profile, linear regression analyzes were performed for continuous variables and logistic regression analyzes for categorical variables. All statistical tests were considered with a significance level of 95% (p <0.05). In the population, 91 were boys and 83 were girls, in which 55.1% of boys and 44.9% of girls had altered ALT. Boys had a higher level of physical activity (p=0.032), higher caloric contribution from the diet (p=0.011), higher serum values of AST (p=0.003) and phosphatase (p<0.001) and girls had higher consumption of fructose from from the group of processed foods in the diet (p=0.043). Fructose consumption from group 3 was associated with an increase in BMI (B=0.517; 95%CI: 0.021-1,013), WC (B=8,131; 95%CI: 0,523-15,740) and HOMA-IR (B=4,337; 95%CI: 0,528-8,147) and decrease in HDL-c values (B=5,246; 95%CI: 1,098-25,076). After adjusting for energy, sex, age, income and waist circumference, only HOMA-IR remained associated. Furthermore, group 4 fructose was associated with a higher risk of an increase in triglycerides (B=1386; 95%CI: 1822), regardless of energy, sex, age, income and waist circumference. With this, it is evidenced that the consumption of fructose from the groups of processed and ultra-processed foods were related to metabolic alterations in adolescents with excess weight, however, no association was found with alterations in ALT concentrations.
