SUMMARY:

The number of People with Disabilities (PwD) has continuously increased in Brazil. This scenario is related to population aging, increase in Chronic Non-Communicable Diseases (NCD), high rate of traffic accidents and firearms, technological advances in intensive care and emergency, as well as relation to infectious diseases. PwD, in turn, are more vulnerable and tend to have more unfavorable health outcomes due to the barriers they face, resulting in inequality of access to education, employment, health care, quality of life, and consequently food insecurity (FI). Objective: To understand the dimension of food and nutritional insecurity, quality of life, and health conditions of PwD in the Brazuca Natal research. Methodology: Hybrid study, considering a cross-sectional and case-control study, based on the BRAZUCA Natal Study. The cross-sectional study includes 39 people (16 adults and 23 elderly) with disabilities. For the control case, 115 people (58 adults and 62 elderly). Data collection was carried out in person between August 2019 and March 2020, being suspended due to the COVID-19 pandemic. Demographic, socioeconomic, lifestyle, health conditions, and nutritional status data were collected. The Brazilian Food Insecurity Scale (EBIA, in Portuguese), Quality of Life (QL) questionnaires from the World Health Organization – WHOQOL-BREF and WHOQOL-OLD were applied, and anthropometric measures were measured. Results: Physical disability was the most frequent (48.7%). AI was identified in 56.4% of the population, 81.6% did not have a paid job, 86.8% reported not drinking, and 78.6% did not smoke, and 44% were overweight. Regarding the difficulty in purchasing food, 63.2% (CI 47.4% – 78.9%) reported high/extreme difficulty for industrialized products and 57.9% (CI 42.1% – 73.7%) reported food fresh. High proportion had difficulties eating in restaurants (47.4%; CI 31.6% – 63.2%); to store (52.6%; CI 36.8% – 68.4%); and to prepare food (60.5%; CI 44.7% – 76.3%). While no or low difficulty was found to identify foods inappropriate for consumption (63.2%; CI 47.4 – 78.9); putting food on the plate (50%; CI 34.2 – 65.8); eating solid foods (55.3%; CI 39.5 – 71.1); pasty foods (97.4%; CI 92.1 – 100); use a fork and knife (62.2%; CI 47.4 – 78.9) and fill glasses and bottles (47.4%; CI 31.6 – 63.2). Most reported the need for caregivers or family members to buy food (86.8%). When comparing cases and controls, the occurrence of AI was 33.3% in PwD and 66.7% in non-PwD (p=0.140). Among the PwD there was a higher proportion of severe FI (59.3%). Conclusion: PwD has low autonomy to buy and prepare their food, which can impact food choices and unhealthy habits. PwD did not show higher proportions of FI compared to non-PwD in the population evaluated. The QoL of PwD was low, and when compared to people without disabilities, we observed that the physical domain showed the greatest mean difference, putting PwD in a worse condition.
