ASSOCIATION BETWEEN STANDARDIZED PHASE ANGLE AND 12-MONTH MORTALITY OF PATIENTS AFFECTED BY ACUTE MYOCARDIAL INFARCTION: A COHORT STUDY

SUMMARY:

Cardiovascular diseases (CVDs) underscore themselves as the main cause of death in the world, and this includes coronary diseases. Amongst coronary diseases, the acute myocardial infarction (AMI) is defined as myocardial necrosis in a consistent clinical condition with myocardial ischaemia. It is an acute cardiac event with an impact upon the health condition, and its risks factors are usually a cobination of the use of tobacco, inadequate diet, obesity, and sedentary lifestyle, besides to pre-existing comorbidities. These risks factors can compromise the celular integrity impacting on physiological and nutritional components. In this approach, the phase angle (FA), has measured by electrical bioimpedance (BIA), identifies the quality of the cell membrane and the distribution of body fluids. There are previous evidences that this biomarker has a high predictive power of negative clinical outcomes, such as mortality and hospital readmission in several clinical situations. However, it has not been evaluated yet the association of FA with adverse events in patients who were recently affected by AMI. Therefore, our work aims to check if the FA is predictor of mortality and adverse clinical events in 12 months of pacientes who were recently affected by AMI. It has been done a prospective coorte study with a sequence of 12 months. Furthermore, it has been included adults and elderly patients with both sexes who have been admitted to the University Hospital Onofre Lopes (UHOL/FURN) with a diagnosis of AMI. The patients has been avalueted during the hospital internment, and it has been collected demographic, clinical, and nutritional data. Body mass, height, and calf circumference have been measured using the anthropometric technique. The strength of the handshake has been measured with a manual dynamometer. The FA has been calculated through the measuring of the resistance (R) and reactance (Xc) of the bioimpedance, and it has been adjusted based on sex and age, presenting, therefore, the Standardized FA (FAS). All the patients have been followed for a period of 12 months, and the occurrence of adverse events (mortality, hospital readmission, new AMI) has been observed. The sample has consisted of 153 patients with a medium age of 61.2 ± 12.6 years, it being 57.5% elderly. Patients who have presented the standardized phase angle below the 10th percentile have had a shorter time for the occurrence of death (p = 0.024) and for the outcomes in a combined way (p = 0.004). In binary logistic regression analysis, when it has been adjusted for confounding factors, the low FAS has not been associated with adverse events. In conclusion, we have noticed that the low FAS has been associated with the risk of death and occurrence of adverse events in patients who have been affected by AMI. However, when it adjusted for confounding factors this risk is not maintained.
