FOOD CONSUMPTION AND DIETARY INFLAMMATORY INDEX ASSOCIATED WITH HISTOPATHOLOGICAL PROGNOSIS IN PATIENTS WITH THYROID NEOPLASIA IN THE STATE OF RIO GRANDE DO NORTE

SUMMARY:

The National Cancer Institute estimates the diagnosis of 13,780 new cases of Thyroid Cancer (CT) each year of the 2020/2022 triennium, 1,830 in men and 11,950 in women. Dietary factors such as the ingestion of certain nutrients can bring benefits in decreasing the risk of TC or in its severity, playing a fundamental role in the initiation, progression and malignant conversion of the tumor. This study aimed to identify the association between nutrient intake, dietary inflammatory index and tumor histopathological staging (angiolymphatic invasion, tieroid extension, lymph node metastasis and multicentricity) of patients with thyroid cancer in the state of Rio Grande do Norte. This is a cross-sectional study. The sample was established for convenience (between the years 2017 and 2018) and included individuals assisted in the League Against Cancer, who underwent total or partial thyroidectomy for suspected thyroid malignancy. To assess food consumption and the Dietary Inflammatory Index (IID), a 24-hour recall (R24h) was applied, collected in three moments. The cytological and histopathological analyzes and classifications of the tumors were performed at the Pathology Laboratory of Hospital Luiz Antônio (HLA) by pathologists. Significant associations were found between the intake of cholesterol (p = 0.024), iodine (0.026), omega 3 (0.031) and vitamin D (0.032) with the presence of malignant nodules; Vitamin D (p = 0.031) and omega 3 (p = 0.039) in relation to the presence of angiolymphatic invasion; Calcium (p = 0.008) and caffeine (p = 0.027) with extrathyroidal extension; Vitamin E (p = 0.029) and the presence of lymph node metastasis, and Vitamin B6 (p = 0.026) and the presence of multicentricity. Regarding the IID, no associations were found with the histopathological prognosis of CT. Our study is the first to assess the association between IID and CT in Brazilian patients in the Northeast region. These results justify the need for further investigations and, if confirmed, may have important implications for the histopathological prognosis of individuals with TC in relation to food consumption.
