SUMMARY:

Food intake plays a crucial role in the control of heart failure (HF). However, there is
a scarcity of studies that indicate the association between micronutrients intake and
clinical outcomes of HF. The objective of this study was to assess the prevalence of
inadequate nutrient intake and associations with clinical outcomes in outpatient
individuals with HF. The descriptive study of a prospective longitudinal design was
carried out with a sample of 124 adults and elderly people seen at a referral clinic for
care for HF in Natal / RN. Socio-demographic, clinical, anthropometric and food
consumption data were collected through 24-hour recalls. The usual consumption of
energy, macro and micronutrients was evaluated, and the prevalence of inadequate
nutrient intake was calculated. The participants were divided into groups of moderate
and high deficiency, according to the individual assessment of the consumption of
each micronutrient. Clinical outcomes (hospitalizations and mortality) that occurred
at 24 months of follow-up were considered for assessments of overall and diseasefree
survival. Survival was calculated using the Kaplan-Meier estimate and
comparing the curves with Log-Rank. The risk of death rates (Hazard Ratio, HR)
were calculated using Cox regression models. There was a prevalence of 100% of
inadequate consumption of vitamins B1 and D, and greater than 80% for vitamins
B2, B9 , E, calcium, magnesium and sodium. There was no difference in overall
survival and event-free survival for individuals in the groups with moderate and high
micronutrient deficiency (HR = 1.02; p = 0.96 and HR = 1.04; p = 0.92, respectively ).
Overweight, New York Heart Association (NYHA) I or II functional class and higher
glomerular filtration rate showed significant associations with better overall survival
(all p <0.05). Individuals with adequate vitamin A consumption had a longer eventfree
survival compared to individuals with inadequate consumption (HR = 2.22; p =
0.04). Cox regression analysis indicated that NYHA functional class III / IV (p =
0.009) and adequate weight according to BMI (p = 0.035) were independent
predictors of overall survival time. On the other hand, NYHA functional class III / IV
(p <0.001), and inadequate vitamin A consumption (p <0.05) were independently
associated with event-free survival. It is concluded that individuals with HF treated at
the outpatient clinic have high prevalence of inadequate nutrient intake. Inadequate
consumption of vitamin A, eutrophic BMI and NYHA functional class III / IV were
associated with unfavorable clinical outcomes, highlighting the importance of
monitoring these parameters in the context of HF.
