INFLAMMATORY PROFILE OF INDIVIDUALS WITH DUCHENNE MUSCULAR DYSTROPHY: CORRELATIONS BETWEEN CLINICAL AND NUTRITIONAL PARAMETERS

SUMMARY:

Introduction: Duchenne Muscular Dystrophy (DMD) is the most frequent and severe of the dystrophies. Chronic inflammation appears as an important feature of its pathophysiology and progression, with cytokines being biomarkers of systemic inflammation and indicative of muscle damage in DMD. Objective: identify the inflammatory profile of individuals with DMD and evaluate associations between clinical and nutritional variables. Methods: a prospective study was carried out with data collection carried out between January 2018 and June 2019, in the Northeast region of Brazil. Male patients with a confirmed diagnosis of DMD were included. Sociodemographic and disease duration data were obtained by interview; the use of medications was assessed based on information from the patient's medical record; the nutritional status was assessed according to the body mass index (BMI) and the body composition determined with the aid of dual emission x-ray densitometry (DXA). Plasma levels of cytokines IL-1β, IL-6 and TNF-𝛼 were measured using the ELISA test. The relationship between disease duration, cytokines, nutritional status and body composition was assessed by the Wilcoxon test. Spearman's correlation coefficient was used to establish a relationship between cytokines and time of onset of the first signs of DMD and body fat. Then, the evaluation of the prediction of these last two variables on plasma levels of cytokines was performed using multiple linear regression. Results: 45 individuals aged 4.3 to 24.4 years were evaluated. At the time of data collection, individuals had been for 7.9 (7.2 - 10.2) years since the onset of the first signs of the disease. In relation to nutritional status, 17.8% of the individuals presented marked thinness, thinness or underweight, 51.1% eutrophic and 31.1% risk of overweight, overweight or obesity. The percentage of body fat evaluated using DXA showed values of 35.1 (27.4 - 39.0)%. The percentage of body fat increased (p <0.001) over time since the beginning of the first signs of DMD. The values of IL-1β, IL-6 and TNF-𝛼 were, respectively, 21.0 (22.2 - 33.3) pg / mL, 15.1 (14.4 - 21.0) pg / mL and 85.1 (84.2 - 91.4) pg / ml. It was found that cytokines were not related to disease duration. IL-6 correlated with disease duration (ρ = 0.372 p = 0.013). BMI / Age, in turn, correlated with IL-1 β and TNF- 𝛼 (ρ = -0.357 p = 0.030 and ρ = -0.353 p = 0.032), while the percentage of body fat was related to IL- 6 and TNF- 𝛼 (ρ = 0.321 p = 0.041 and ρ = -0.435 p = 0.005). The regression models showed higher values of IL-1β (β = -0.573 p = 0.001) and TNF-α (β = -0.498 p = 0.006) associated with lower percentages of body fat. Our results show the persistent inflammatory profile presented by individuals with DMD, verified by the levels of pro-inflammatory cytokines. From these results, it becomes possible to outline more specific treatment goals, aiming to neutralize the activity of IL-6 as a way to reduce the inflammation presented by these individuals.
