RESUMO:

Thyroid cancer (TC) is the most common malignancy of the endocrine system. According to last estimates from the Brazilian National Cancer Institute (INCA), this neoplasm is expected to reach 9610 people in Brazil by the end of 2018, and the largest number of cases is expected to occur in the Northeast region (Ceará, Paraíba and Rio Grande do Norte). Papillary thyroid carcinoma (PTC) accounts for about 70% of TC cases, and among the modifiable risk factors related to this disease are body composition, eating habits, lifestyle and biochemical status of micronutrients. Experimental and clinical studies have shown that antioxidant vitamins can inhibit the development and progression of cancer, thus the biochemical status of vitamins A and E have emerged as risk factors for the disease. Therefore, the present study investigated the association between body composition, dietary habits, lifestyle, and serum vitamin A and E concentrations with predictors of histopathological prognosis in PTC patients. An evaluation of body composition by tetrapolar bioimpedance device and consecutive nutritional assessment was performed. We also evaluated weekly eating habits in the last 12 months using a Food Frequency Questionnaire (FFQ) and lifestyle habits. Serum vitamin A and E were measured by liquid chromatography in three times: one before thyroidectomy and two after thyroidectomy of PTC patients. The results showed that weekly dietary eating habits couscous/crumbs and/or margarine/vegetable cream over 1x/week may increase the risk of CPT patients present extrathyroidal extension (p = 0.021 and 0.004, respectively), as well as the ingestion of bovine meat over 1x/week may increase the risk of lymph node metastases (p = 0.046). Regarding serum concentration of vitamins A and E, reduced serum levels of vitamin A may be associated with angiolymphatic invasion and lymph node metastases (p = 0.027 and 0.015, respectively) and patients with extrathyroidal extension or angiolymphatic invasion tend to present lower concentrations of vitamin E in post-thyroidectomy compared to preoperative moment.
