RESUMO:

The objective of this study was to evaluate the quality of the meals consumed by institutionalized elderly. A cross-sectional study was carried out with 322 elderly people living in ten nursing homes in Natal, Brazil, between 2013 and 2014. Bio-sociodemographic, nutritional status and food consumption data were collected. In order to evaluate the quality of the meals, the Meal Quality Index (MQI), developed by Bandoni and Jaime in 2008, compound by five components (Adequacy of vegetables, fruits and vegetables; Carbohydrate; Total Fat, Saturated Fat and Meal Variability), and the Main Meal Quality Index (MMQI), proposed by Gorgulho and collaborators in 2015, with ten components (Fruits, vegetables, fiber, animal protein / total protein, carbohydrates, total fat, saturated fat, processed meats, sweetened drinks and desserts, energy density) were calculated. The maximum score of both is 100 points. The classification of the two indexes was adapted for the elderly and used in consumption (instead of supply) of nutrients and foods. Variables of the indices do not diffuse normal distributions. The significance level of 5% was used for all statistical tests. Descriptive analysis of the population was performed by absolute and percentage frequencies and IQRP by central tendency, dispersion and percentiles. All components of the index were analyzed separately for three main meals. To compare IQRP medians between as non-profit / non-profitable nursing homes, the Mann-Whitney test was used for independent samples. To assess agreement between the two meal quality indices, perform the Wilcoxon test. The scores of the two indices were categorized into quartiles and the concordance between them for cross-graded and scored. The proportion of old people categorized in the same quartile, same and adjacent quartiles, and quartiles opposing (discordance) were calculated. The population of the study was predominantly female, residing in non-profit nursing homes and more than 80 years. The nutritional status of the elderly showed a large proportion of low weight. The best quality meal was the lunch, with good scores of saturated fat, processed meats and sweetened drinks and desserts. There is evidence of poor consumption of vegetables and fiber and energy density at all meals. The quality of the meal consumed in the for-profit ILPI was better than in the non-profit. A cross-classification by quartiles showed good agreement, being an exact agreement of 46.6% and a disagreement of 3.1% (weighted Kappa = 0.402). The proportion of elderly in the same quartile or adjacent quartile was more than 80%. The use of Meal Quality Indexes makes it possible to evaluate several aspects of the meal of fundamental importance in the quality of food consumed by the elderly.
