Concerns about Body Image (BI) are very common among women. However, women deprived of their liberty are exposed to conditions of life different from those in freedom, which can affect their body perception in a distinct way. The present study aimed to evaluate BI and factors associated with BI distortion and dissatisfaction in women incarcerated in a closed regime in Natal/RN. This cross-sectional study was carried out with detainees of the Dr. João Chaves Female Penal Complex (n = 107), aged between 18 and 57 years. The body image was conducted by the Brazilian Figure Rating Scale (FRS). Personal sociodemographic information (age, schooling, family income, marital status, confinement time, frequency of visits and number of children) was collected by interview, and weight and height were measured by means of anthropometry, and then calculated the Body Mass Index (BMI). The Prevalence Ratio and their respective 95% Confidence Interval were used to evaluate the association between the dependent variables (distortion and dissatisfaction of BI) and the independent variables. The differences between means of BMI measured by anthropometry (BMI-A) and that estimated by FRS (BMI-S) were verified by Student's t-test. The concordance between BMI-A and BMI-S was verified by the Bland-Altman method. Study participants had a mean age of (31,7 + 8,48 years), and the mean BMI-A was 27,67 Kg/m². About 65% were overweight, of which 27,8% were obese. There was a distortion of BI in 83,3% of participants. Of these, 82,7% overestimated the size of their body, attributing an average of 5,59 kg/m2 (+ 2,73) more to their real body. Most women (91,6%) were dissatisfied with their bodies and wanted a different body than they had. Although most wanted to have a BMI, on average, 9,01 kg/m2 (+ 5,25) lower, about one-third wanted to increase body size by an average 6,43 kg /m². Sociodemographic characteristics, nutritional status and confinement time were independently associated with distortion and dissatisfaction with BI. Most female inmates are dissatisfied with their bodies and distort their BI.
