Hypovitaminosis D has been a frequent finding in renal patients, and in transplanted individuals, may cause progression of albuminuria, potentiating the decline in renal function and the risk of graft loss. The objective of the study is to evaluate the vitamin D status in subjects submitted to renal transplantation at zero time, 3 months and 6 months post-transplantation. This is a longitudinal study developed undergoing renal transplantation, attended at the Ambulatório de Nefrologia of the Hospital Universitário Onofre Lopes (HUOL) of the Universidade Federal do Rio Grande do Norte (UFRN). The subjects were evaluated at zero time, 3 and 6 months post-transplantation, being submitted to anthropometric evaluation, solar exposure, food consumption, biochemical evaluation, 25-hydroxyvitamin D [25(OH)D] urine collection to evaluate a albumin:creatinine ratio (RAC). In relation to statistical analyzes, the values were expressed as median (interquartile range). The Kolmogorov-Smirnov test was performed to verify the normality of the quantitative variables, and was applied Mann Whitney tests for comparisons between groups. For the correlation analyzes, Spearman's tests were used, and the square test was used to test as categorical variables. Multiple regression analyzes were applied to identify the variables that constitute an independent association with a RAC. We studied 28 individuals with a median age of 47 years (ranging from 35 to 57 years), the majority males gender (n = 16; 57,1%) and brown/mulatto ethiny (n = 22; 78,6%). There was no statistical difference between the values of the Body Mass Index (BMI) and between the groups of nutritional status classification between the three times, predominating a classification of eutrophy at all times. The variables related to food consumption were also not statistically different. Consumption of energy, carbohydrates, lipids and vitamin D remained below recommended, while protein and fiber are in excess. An improvement in the glycemic profile and worsening of renal function and lipid profile were observed throughout the follow-up. There was a predominance of subjects with 25(OH)D values classified as insufficient the three times studied, and individuals with vitamin D deficiency showed a greater tendency of renal function changes, total cholesterol, LDL-c, and parathyroid hormone (PTH). LDL-c and PTH maintained an independent association with a post-transplant RAC, and 25(OH)D correlated negatively with RAC without time zero (r = -0.612; p = 0.045). It was possible to conclude that the alteration in vitamin D status may be related to the renal function of the individuals submitted to renal transplantation, independent of the post-transplant period. In addition, it is necessary to plan interventions that aim to correct the vitamin status in order to obtain better renal outcomes.
