RESUMO:

Metabolic syndrome (SM) is a multifactorial disease, whose prevalence is influenced by genetic and dietary factors. Zinc plays special interest considering their biological functions related to the pathogenesis of SM. The aim of this study was to evaluate the status of zinc and associations with cardiometabolic and nutritional factors in adults and  elderly with SM. This is a case-control study, developed with adults and elderly assisted in the Endocrinology clinic of the University Hospital Onofre Lopes (HUOL). SM group consisted of 88 participants characterized according to NCEP/ATP-III criteria. Control group consisted of 37 individuals without SM or other clinical condition with influence on parameters of zinc. Anthropometric assessment was performed and dietetics, through 24-hour food Recall. Blood samples were collected to assessment of lipid profile, glucose, ultra-sensitive C-reactive protein, plasma and erythrocyte zinc. Urine was collected for analysis of zinc excretion. Were applied "t" Student's test and Fisher's exact test for evaluation the differences between groups. Data have been adjusted considering covariates, through simple linear regression models and linear multiple regression models. Correlations were evaluated by Pearson coefficient (r), considering a significance level of 5%. The average age of participants was 50 (± 11) years and 44 (± 11) years for SM group patients and controls, respectively, with female predominance in both groups. In SM group were observed significantly higher values of systolic and diastolic blood pressure, fasting blood glucose, HDL-c (p<0.001) and triglycerides (p=0.034), with the exception of HDL-c concentrations were significantly lower in comparison to the control. It was also reported significantly increased values in SM group of insulin, HbA1c (p<0.001) and HOMA-IR (p=0.002) and PCR-HS (p<0.001).The average daily caloric intake was significantly higher for patients with SM (p = 0.003) and dietary intake of both groups was characterized as hyperproteic, adequate in carbohydrates and lipids. The average consumption of zinc in SM group was significantly lower (6.57 ± 1,64mg/day vs 9,37±2,41mg/d; p<0,001), with prevalence of inadequacy over 40% for both groups. There were no significant differences in the values of plasma zinc (p= 0.783) between the groups. Significantly higher concentrations identified in erythrocyte zinc (p < 0.001) and urine zinc (p =0.008) in SM group compared to the control, independent of the covariate adjustments. The SM group positive correlations were observed with the waist circumference (r = 0.253; p = 0.02), triglycerides (r = 0.360; p < 0.01), fasting glucose (r = 0.479; p < 0.01), HbA1c (r = 0.250; p = 0.02) and PCR-HS (r = 0.427; p < 0.01).Same result not found in the control group, with the exception of waist circumference(r=0.434; p<0.01).
