SUMMARY:

Heart failure (HF) is a complex clinical syndrome frequently associated with alterations in body composition, including obesity, sarcopenia, sarcopenic obesity, and cachexia, which impair functional capacity and prognosis. However, few studies have characterized these phenotypes using objective and accurate methods, such as dual-energy X-ray absorptiometry (DXA). This study aimed to analyze body composition according to different sarcopenia and adiposity phenotypes and their associations with functional performance in patients with HF. A cross-sectional study was conducted using a convenience sample of 74 individuals with a previous diagnosis of HF. Sociodemographic and clinical data were collected, and body composition was assessed using DXA. Muscle strength was evaluated by handgrip strength and specific muscle strength (SMS). Functional performance was assessed using the five-repetition sit-to-stand test (5STS) and the six-minute walk test (6MWT). The primary outcome was abnormal functional performance assessed by the 5STS. Associations between isolated and combined criteria of obesity, sarcopenia, and sarcopenic obesity with the 5STS were analyzed using Poisson regression models adjusted for age, sex, and left ventricular ejection fraction (LVEF) classification. As an exploratory secondary outcome, 6MWT distance was analyzed using Pearson’s correlation and linear regression. The total sample (n = 74) was predominantly male (64.9%), with a mean age of 58 ± 11.9 years, and reduced LVEF (54.05%). Abnormal 5STS performance was identified in 24.32% of patients. Low handgrip strength (63.2%) and obesity defined by elevated body fat percentage (58.1%) were the most prevalent conditions, while sarcopenia (20.3%) and sarcopenic obesity (12.2%) also showed relevant frequencies. Patients with abnormal 5STS performance had a higher proportion of low appendicular lean mass index (19.64% in normal 5STS vs. 44.44% in abnormal 5STS; p = 0.036), as well as a higher frequency of reduced specific muscle strength (25% vs. 50%; p = 0.046). In adjusted models, higher handgrip strength values were independently associated with better 5STS performance [PR = 0.94 (95% CI 0.90–0.99); p = 0.028], whereas sarcopenia and sarcopenic obesity significantly increased the prevalence of functional impairment assessed by the 5STS [PR = 2.23 (95% CI 1.08–4.60); p =0.030 and PR = 2.75 (95% CI 1.45–5.23); p = 0.020, respectively]. Each 1-kg increase in handgrip strength was associated with a 3.15-m increase in 6MWT distance (p = 0.050) and a 0.25-s reduction in 5STS time (p = 0.008), remaining the only variable significantly associated in complementary models. In conclusion, handgrip strength is an independent marker of functional performance in outpatients with HF. Sarcopenia and sarcopenic obesity were associated with a higher likelihood of functional impairment, reinforcing the importance of integrated assessment of body composition and functional capacity in the clinical management of HF.
