SUMMARY:

The food consumption markers from the Brazilian Food and Nutrition Surveillance System (SISVAN) have shown good potential to reflect overall diet quality. However, they have not yet been used to assess the diets of women with gestational diabetes mellitus (GDM). It is well established that good adherence to nutritional therapy improves glycemic control, reduces maternal and fetal hyperglycemia, decreases obstetric and neonatal complications, and regulates maternal and fetal weight gain, highlighting the importance of conducting this type of assessment in specialized public health care. Therefore, the aim of this study was to examine the association between adherence to healthy diet markers and obstetric and neonatal glycemic control among women with GDM receiving care at the high-risk prenatal clinic of Maternidade Escola Januário Cicco. This was a cohort study involving women with GDM between the 14th and 32nd weeks of pregnancy, followed at three time points (T1, second trimester; T2, third trimester; and T3, immediate postpartum). Sociodemographic and clinical information, anthropometric data, prenatal glycemic control, dietary intake, and postpartum obstetric outcomes were collected. Adherence to dietary markers was assessed using scores for healthy and unhealthy eating markers adapted from SISVAN. Maternal glycemic control was evaluated through capillary glucose levels at fasting, post-breakfast, post-lunch, and post-dinner obtained from self-monitoring, as well as by reductions in blood glucose between T1 and T2. Neonatal outcomes included the presence of neonatal hypoglycemia. By the time of the qualification stage, frequent consumption of beans (82.1%), vegetables (70.0%), and fruits (61.0%) was observed, alongside a high intake of sugar-sweetened beverages (58.4%) and instant noodles/snacks (45.8%), indicating a mixed dietary pattern. Neonatal hypoglycemia occurred in 31.1% of infants and was associated with higher consumption of unhealthy dietary markers (p = 0.026). In addition, a trend was identified for greater maternal glycemic variability in self-monitoring with higher adherence to unhealthy markers, but also for daily glycemia variation with healthy markers. Despite the good association between SISVAN food consumption markers and macro- and micronutrient intake, these findings suggest that SISVAN markers should be interpreted with caution in the context of GDM.
