SUMMARY:

The food consumption markers of the Food and Nutritional Surveillance System (SISVAN) have shown good potential to reflect overall diet quality; however, it is not yet known whether adherence to them impacts the glycemic profile of women with Gestational Diabetes Mellitus (GDM). It is known that good adherence to a healthy diet improves glycemic control, reduces maternal and fetal hyperglycemia, decreases obstetric and neonatal complications, and controls maternal and fetal weight gain, making it important to conduct this assessment in specialized public health care. This study examined the association between adherence to the SISVAN food consumption markers and the obstetric and neonatal glycemic control of women with GDM treated
at the high-risk prenatal outpatient clinic of the Januário Cicco Maternity Teaching Hospital. This was a cohort study of women with GDM between the 14th and 32nd weeks of pregnancy, followed up at three moments (T1, second gestational trimester; T2, third gestational trimester; and T3, postpartum) and grouped according to treatment into: Control group, treated with lifestyle and diet changes, and Insulin group, those using insulin therapy. Sociodemographic information, clinical information, anthropometric data, prenatal glycemic control, and 24-hour dietary recalls (24HR) were collected, in addition to postpartum obstetric outcomes. Adherence to the markers was verified according to healthy consumption marker (HCM) and unhealthy consumption marker (UCM) scores adapted from SISVAN. Maternal glycemic control was assessed by fasting, post-breakfast, post-lunch, and post-dinner capillary blood glucose resulting from self-monitoring, and by the difference in blood glucose between T1 and T2 (delta). The 190 participants had a mean age of 31.5 years, and 48.4% had a high cumulative weight gain at the end of pregnancy. Frequent consumption of beans (82.1%) was observed; however, almost 1/3 of the women did not intake vegetables and fruits, and showed high intake of sweetened beverages (58.4%) and instant noodles/snacks (45.8%), characterizing a mixed dietary pattern. Adherence to HCMs and UCMs was associated with the dietary macronutrient profile (p < 0.05). In the control group, women who consumed three HCMs showed a reduction in fasting blood glucose between T1 and T2 (β = -5.91 mg/dL; 95% CI: -11.45 to -0.38; p = 0.037; R2 = 0.0865). Women in the insulin group who presented more than three UCMs showed an increase in blood glucose when comparing T1 and T2 (β = 16.24 mg/dL; 95% CI: 7.03 to 25.45; p = 0.001; R2 = 0.2235). However, post-lunch and post-dinner blood glucose levels increased according to adherence to HCMs. Neonatal hypoglycemia occurred in 31.1% of the babies and was associated with a higher
consumption of UCMs (p = 0.026; R2 = 0.002). Despite a good association between food consumption markers and macro- and micronutrient intake and glycemic profile, the results with glycemic self-monitoring suggest that the SISVAN food consumption markers should be evaluated with caution in GDM.
