Cardiovascular diseases (CVD) remain the leading cause of global mortality, with acute myocardial infarction (AMI) standing out as a major contributor. Recent evidence reinforces the protective role of healthy dietary patterns, predominantly plant-based, in reducing cardiovascular risk. In the post-AMI context, diet quality plays a central role in preventing adverse outcomes, such as cardiac remodeling and left ventricular systolic dysfunction, conditions that may progress to heart failure. The aim of this study was to evaluate the influence of diet quality on changes in left ventricular ejection fraction (LVEF) over one year following AMI. A cohort study was conducted with post-AMI patients, in which diet quality was assessed using the Global Diet Quality Score (GDQS), calculated from 24-hour dietary recalls. Changes in LVEF between 6 months (T1) and 12 months (T2) post-event, according to GDQS tertiles, were analyzed using linear mixed models adjusted for potential confounding factors. The analyzed sample presented a mean GDQS of 23 (SD = 3.4), with higher consumption of whole grains, liquid oils, eggs, and poultry compared with other studies in the Brazilian population, while consumption of nuts, seeds, and orange-colored vegetables was lower. Individuals who had never smoked and those who engaged in physical activity according to World Health Organization recommendations showed better diet quality scores. Individuals in the highest and lowest GDQS tertiles showed a similar trajectory of LVEF changes over the follow-up period. These findings highlight that recovery of cardiac function after AMI depends on multiple factors beyond diet quality and reinforce the effectiveness of current treatments combined with strategies to promote quality of life and nutrition education as part of secondary prevention.
