SUMMARY:

The double burden of malnutrition is a condition that has been growing in several regions of the world, where negative health outcomes affect mothers and children, characterized by excess maternal weight and nutritional deficiencies. In this context, assessing diet quality can be a low-cost indicator for assessing risk factors for malnutrition. This study aims to evaluate the association between diet quality and the occurrence of malnutrition and double burden of malnutrition in the mother-child binomial. Therefore, this study presents two methodological designs: a systematic review (SR) and a cross-sectional review. The SR was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, registered with PROSPERO (CRD42024580086). The search was conducted in the PubMed/MEDLINE, Lilacs/SciELO, Embase, Scopus, Web of Science, and Google Scholar databases. The eligibility criteria for inclusion of the studies involved the analysis of the relationship between maternal diet quality and maternal and/or child malnutrition (overweight, obesity, weight retention, nutritional deficit) up to two years postpartum. The cross-sectional study was conducted with 131 pairs of lactating women and their children between 30 and 150 days postpartum at Primary Health Care Units in the city of Natal, Rio Grande do Norte, from July 2021 to March 2023. Socioeconomic, anthropometric, and maternal food consumption data were collected through the 24-hour dietary recall. Diet quality was assessed through the contribution of ultra-processed foods (%), Global Diet Quality Index (GDQS), and Minimum Dietary Diversity Score for Women (MDD-W). The double burden of malnutrition was defined by maternal excess weight (overweight or obesity) and nutritional deficits in children according to the growth curve. In the eight articles included in the RS, most of which were cross-sectional, 13 different indicators of diet quality were found, with overweight/obesity in women and stunting in children being the main factors, with a significant association between maternal diet quality and the presence of child malnutrition. The double burden of malnutrition was not studied. In the cross-sectional study, most women were overweight (70.2%), and the prevalence among children was 0.8% with high weight for age, 13% with low or very low weight for age, 9.9% were at risk of overweight, overweight, or obesity, 10.7% were thin or severely thin, 3.8% with low or very low height for age, and 17.6% with a double burden of malnutrition. The %AUP compared to the different nutritional statuses ranged from 15.99% to 24.47%, with the diet indicating a moderate risk of nutritional inadequacy according to the GDQS and a minimum adequate dietary diversity according to the MDD-W. No statistically significant differences were observed. There is a scarcity of studies on the relationship between maternal diet quality and malnutrition in the mother-child binomial, with few findings associating inadequate diets with the risk of childhood stunting. No significant associations were observed with the occurrence of malnutrition and double burden. Further statistical analyses will be conducted.
