Introduction: The pregnant’s card is an important recording instrument that must always be in the pregnant woman's possession. All assistance to the pregnant woman must be duly registered in the card, since this record, as well as its quality, reflects the practice of care and serves as parameters to guide the following decisions. Through such registry it is also possible to monitor the follow-up of guidelines for prenatal care and to identify policy needs that improve the quality of care. Objectives: To evaluate the completion of the card/booklet of the pregnant woman who were followed during the prenatal care to postpartum in municipalities in the Trairi Region of the state of Rio Grande do Norte. Methods: This analysis was conducted by an evaluative, exploratory, and descriptive research, and also using quantitative methods. The sample is composed by women followed during the prenatal care in five cities from the Trairi Region, in the state of Rio Grande do Norte, Brazil: Santa Cruz, Lajes Pintadas, Tangará, Campo Redondo e São Bento do Trairi, and who are part of the AMOR project (Adolescence and Motherhood Research). Data collection followed a standardized protocol: evaluation of sociodemographic data; evaluation of the pregnant’s card in relation to the completion of the information and to the following of recommendations of the Brazilian Ministry of Health. Results: The current sample is composed by 40 women, and present a higher proportion of adolescents (57,5%), married women or in a stable union (75,0%), who have completed high school (90, 0%) and who have not planned the pregnancy (45,0%). All the 15 categories analyzed in this study failure in relation to the record, especially in terms of filling the frame about “Folic acid supplementation” and “participation in educational activity”, that have had the lowest percentages (5,0% and 5,4%). The ABO/Rh exams, as well as Ultrasound in Obstetrics and Gynecology (UOG) were accomplished by all women of the sample, but no item among the strategic indicators of the Rede Cegonha reached a proportion of 100%. The hemoglobin/hematocrit tests were held in higher proportion by the sample (36,0%), in accordance with the pregnancy period recommended by the Rede Cegonha, and the urine culture test was the least accomplished by the pregnant women in the sample, with a percentage of 22.5%. Conclusion: It is noteworthy to observe that the adequate completion and follow-up of the recommendations established during prenatal care are still fragile despite the numerous efforts of the Ministry of Health to offer a humanized, integral and resolutive assistance that will achieve the objectives of reducing high mortality rates maternal and child health.
