Introduction: Aging is a natural, dynamic and irreversible process characterized by the decline of body systems, including loss of bone mass, muscle mass and strength. Evidence shows that, with aging, women present worse physical performance compared to men, suggesting that there are factors related to gender, such as variables of reproductive history, that may explain such differences. High parity and early age at the first child are related to the occurrence of urogynecological disorders, such as Urinary Incontinence (UI) and Pelvic Organ Prolapse (POP). It is hypothesized that such disorders are related to the worse physical performance in women, however this is a subject still under investigated. Objective: To evaluate the association between physical performance, urinary incontinence and pelvic organ prolapse in elderly women living in five different cities. Methodology: This is a cross-sectional study derived from the International Mobility in Aging Study (IMIAS), conducted in the cities of Saint–Hyacinthe (Quebec, Canada), Kingston (Ontario, Canada), Manizales (Colombia), Tirana (Albania) and Natal (Brazil). In IMIAS, community-dwelling older women (65-74 years-old) were assessed in the years 2012, 2014 and 2016. This study shows the data collected in 2014. Socioeconomic data, anthropometric measures and reproductive history were collected. Physical performance was assessed using the Short Physical Performance Battery following a standardized protocol that measures balance, gait speed, and lower limbs strength. The SPPB total score was used to classify performance as good (score ≥8) or poor (<8). Urinary incontinence in the past 7 days was assessed by self-report, and classified as “none, once, more than once”. The presence or absence of pelvic prolapse was evaluated by means of self-report, as well as the degree of discomfort caused by it. Bivariate analysis was tested using the Student's t test and chi-square test, binary logistic regression (categorical outcome) and multiple linear regression (quantitative outcome) for assessment the relationship between urogynecological variables and physical performance in SPPB, adjusted for covariates (age, city of study, income, schooling, BMI, reproductive history). The significance level of 5% and 95% CI were considered. Results: The sample was composed by 915 women with mean age of 71.20 (± 2.88), from which 12.5% reported urinary incontinence more than once in the past week and 10.6 only once. In relation to POP, 8.7% reported it and among those, 11.4% reported that it bothers “very greatly”. A higher proportion of women reporting UI have SPPB <8 than those who do not report UI (p <0.001), which is even higher among those reporting frequent UI (more than once a week). This relationship remained even after adjustment for covariates. The women who reported POP had a significantly lower SPPB mean than the others, even after adjusting for age and place of study (β = 0.466, p = 0.045). Moreover, those reporting that POP bothers “very greatly” presented worse values of SPPB compared to those who did not report any discomfort, even after adjustment for covariates (β = 3.089, p = 0.002).
