Health crises increase pressure on healthcare systems and require institutional capacity and adequate funding. In Brazilian federalism, municipalities are responsible for providing health services, although there are differences in fiscal capacity, infrastructure, and human resources. In this context, intergovernmental voluntary transfers play an important role as a financial support mechanism during times of higher demand. Given this, this dissertation analyzes the effects of voluntary transfers on the efficiency of health services in Brazilian municipalities in the context of a health crisis. The analysis is carried out using a  quantitative approach, with panel data and econometric models with municipal and time fixed effects, considering the periods before, during, and after the health crisis. The efficiency of health services is evaluated using indicators related to supply, access, and health outcomes, with voluntary transfers serving as the primary explanatory variable, controlled for socioeconomic, fiscal, and demographic factors. The results did not reject the research hypotheses, revealing significant differences in the efficiency of basic health services between the pandemic and pre- pandemic periods, as well as between the pandemic and post-pandemic periods. The findings also showed that municipalities receiving larger volumes of voluntary transfers demonstrated greater efficiency in basic health services and that, during the COVID-19 pandemic, voluntary transfers had a positive effect on the efficiency of basic health services.
