SUMMARY:

Introduction: Bedside ultrasonography, referred to as Point-of-Care Ultrasound (POCUS), is an effective diagnostic tool for the rapid assessment of critically ill patients. The eFAST (Extended Focused Assessment with Sonography for Trauma) protocol is widely employed to diagnose severe injuries in trauma settings. Despite advancements in POCUS education since the 1990s, its implementation in Brazilian institutions remains limited. Objectives: This study aimed to develop and implement a training model to equip medical students and residents with the skills to perform the eFAST protocol, evaluate their pre- and post-training knowledge, and propose the inclusion of this methodology in the formal curriculum of the Medicine program at the Federal University of Rio Grande do Norte (UFRN). Methodology: A quasi-experimental, before-and-after, analytical, and prospective study was conducted, approved by the Research Ethics Committee of the Onofre Lopes University Hospital (HUOL). A total of 157 participants were divided into three groups: clinical cycle students (N=51) enrolled in the Emergency Medicine course, internship students (N=78) from the Emergency and Urgent Care module, and medical residents (N=28) affiliated with HUOL. Exclusions included participants who did not consent, withdrew, or were unavailable due to institutional activities (N=13). The training consisted of three stages: 1. A 60-minute interactive lecture covering the basic principles of ultrasonography and the eFAST protocol. 2. A 2-hour practical session for learning equipment handling and protocol execution using volunteers. 3. Supervised practice with patients in the Adult ICU at HUOL, exclusively for the residents. Assessments included 11-item questionnaires administered pre- and post-training, with the latter conducted immediately after Stage 1 and following Stage 2. Additional evaluations included retention testing for clinical cycle students, an Objective Structured Clinical Examination (OSCE) for interns, and Direct Observation of Procedural Skills (DOPS) for residents. Satisfaction surveys were also administered at the end of the training. Data were analyzed with a significance level of 5%. Results: All groups showed significant improvement in knowledge following the training (p-value < 0.0001), with post-practical session test scores averaging 8.11 ± 1.65, 8.75 ± 1.12, and 8.79 ± 1.12 for the clinical cycle, internship, and residency groups, respectively. In the clinical cycle group, students who did not participate in the practical session had lower performance in cognitive retention assessments (p-value = 0.015). During the OSCE, interns achieved an average score of 78%, while in the DOPS evaluation, 70% of residents were deemed competent to perform the protocol under supervision, and 30% were considered independently proficient. Regarding satisfaction, 79.4% of participants reported confidence in applying the eFAST protocol, and over 90% positively rated the training model. Conclusion: The proposed model proved effective, significantly improving participant performance in the evaluations. Furthermore, the methodology was well-received by students and residents, making it a promising tool for training in ultrasonography within the context of Brazilian medical education.
