SUMMARY:

Blood flow restriction (BFR) training involves exercise under partial vascular occlusion, with individualized pressure set as a percentage of arterial occlusion pressure (AOP). AOP is influenced by factors such as body position, cuff width, and anthropometric characteristics, but the specific contribution of lower limb tissue composition (TC) remains poorly understood. Predictive equations for 18 cm cuffs represent a practical alternative to direct AOP measurement, yet lacking external validation. This dissertation aimed to examine the relationship between lower limb TC and AOP across different body positions and between sexes; and evaluate the external validity of predictive equations for 18 cm cuffs. Both studies follow a cross-sectional observational design and used the same dataset, comprising 51 healthy adults (25 men and 26 women) recruited through non-probabilistic sampling. Participants completed a single laboratory visit, during which brachial blood pressure, thigh circumference, and body composition via dual-energy X-ray absorptiometry (DXA) were collected. AOP was measured using a portable vascular Doppler, with an 18 cm nylon cuff positioned on the proximal portion of the right thigh, in supine, seated, and standing positions, in random order, with a 5-minute interval between measurements. Two studies were included to comprise this dissertation. The first study investigated the association between lower limb TC (lean mass and fat mass assessed by DXA) and AOP across three body positions, as well as the influence of sex on this relationship. The second study evaluated the external validity of predictive equations for AOP estimation using an 18 cm cuff. The results indicated that TC (lean and fat mass assessed by DXA) is associated with AOP, particularly in the standing position, but loses significance when thigh circumference and systolic blood pressure are considered. Sex was not a relevant predictor. The predictive equations proposed by Wedig et al. (2024) and Cirilo-Sousa et al. (2019) did not demonstrate sufficient external validity, likely due to methodological limitations and population variability. Our results led to the conclusion that predictive equations for assessing lower limb AOP using 18 cm cuffs lack sufficient external validity. Additionally, the relevance of lower limb tissue composition for AOP determination is limited and dependent on body position, being more pronounced in orthostatic positions.
