SUMMARY:

Objective: This study aimed to evaluate the acute effects of high-intensity interval exercise (HIIE) and moderate-intensity continuous exercise (MICE) on immune responses in people living with HIV (PLWH) and healthy adults. Methods: Sixteen participants (8 PLWH and 8 controls) underwent HIIE protocols (both conditions) and/or MICE (only in PLWH), with a crossover design between HIIE and MICE in the HIV condition. Blood samples were collected at baseline, immediately post-exercise (IP), and 1 and 24 hours after exercise. Total immune cell counts (leukocytes, lymphocytes, neutrophils, eosinophils, and monocytes), lymphocyte subpopulations (CD3, CD4, CD8, CD19, and CD4/CD8 ratio), and cytokine production by stimulated lymphocytes (IL-2, IL-4, IL-6, IL-10, IL-17A, TNF-α, and IFN-γ) were analyzed. Results: Significant increases in total leukocyte and lymphocyte subsets were observed IP exercise across all groups, with the HIV-HIIE group showing sustained elevations at 24 hours in leukocytes, eosinophils, monocytes, and neutrophils. CD8⁺ cell counts increased more prominently in the HIV-HIIE group compared to HIV-CME and HIIE-Control. Regarding cytokine production, the HIV-HIIE group exhibited significantly elevated levels of IL-2, IL-4, IL-17A, and IFN-γ at 24 hours post-exercise compared to baseline and the other groups. Conclusion: These findings highlight the immunological impact of high-intensity interval exercise in PLWH, particularly the elevated cytokine production (IL-2, IL-4, IL-17A, and IFN-γ) observed 24 hours post-exercise. This underscores the importance of individualized exercise prescriptions that consider both intensity and immune status.
