SUMMARY:

Abstract

Objective: To evaluate adverse events in pediatric patients and examine their correlation with different vaccine doses.

Methods: The study adhered to PRISMA guidelines, gathering data from multiple sources such as MEDLINE/PubMed, ClinicalTrials.gov, Web of Science, Embase, and Scopus. It included randomized or quasi-randomized clinical trials investigating solicited adverse effects by vaccine dose among children aged 0 to 17 years. Risk of bias was assessed using the Cochrane Risk of Bias tool, and data synthesis was performed using RevMan Web.

Results: Out of the 19 articles included, 15 were analysed in the meta-analysis. Vaccinated showed a higher risk of tenderness and axillary disease after the first dose of protein subunit vaccines (dose: 1st 40.4% RR = 0.83, CI 95% 0.79–0.88), and a higher risk of swelling after the first dose of mRNA vaccines (dose: 1st 0.5% RR=0.68, CI 95%,0.51–0.89). After the second dose, adenovirus vector vaccines were linked to significant local pain (dose: 2nd 3.2% RR = 19.33, CI95%,11.75–31.78). Systemic adverse events like fever, myalgia, vomiting, chills, and drowsiness were noted with the first dose of mRNA vaccines, while irritability and crying were more common with the second dose. Those who received the second dose also faced increased risks of febrile events with inactivated and adenovirus vaccines. The third dose showed similar trends of local and systemic adverse events as the first two doses.

Conclusions: Adverse effects were mild or moderate, short-lived, and occurred with both vaccine doses. Findings suggest benefits of vaccinating, which not only prevents diseases but also contributes to broader community protection.
