Assessment of 100% Rapid Review as an Effective Tool for Internal Quality Control in Cytopathological Services

Objective: Assessment of 100% rapid review (RR) as a tool to analysys the negative Pap smears. Study Design: 8677 swabs were analyzed, the negative results were submitted to RR-100%, the divergent cases were discussed in consensus meeting for conclusion of final diagnosis. Results: RR-100% identified 255 (3.13%) false negative smears (FN) and 552 (6.77%) Unsatisfactory. The RR-100% of the results when compared to the final diagnosis was agreement diagnosis in 7063 (86.60%) and results in 1092 (13.40%) considered discordant. The steepest percentage of discordance was unsatisfactory results, in 1036 (65.61%) cases was as used the Kappa method for RR reliability analysis and final diagnosis with the agreement of 0867 with K = 0.734 (p <0.0001) The sensitivity and specificity with their confidence intervals, positive predictive value (PPV) and negative predictive value (NPV) of RR-100% compared to the final diagnosis were also evaluated. Considering only the cases considered negative sensitivity was 99.1% (95% CI 96.7 to 99.9), specificity 99.4% (95% CI 99.2 -99.6), VPP 84.9% VPN and 100.0%. Conclusion: The result of the RR-100% was considered efficient for FN results when used as IQC method.
