Immunohistochemical evaluation of venous ulcer treated with glucan

Glucan is a polymer of glucose that has been reported to accelerate wound healing through the activation of immune cells. Venous ulcers are cutaneous wounds that result from chronic venous insufficiency; they are difficult to heal and responsible for 80-85% of ulcers that occur on the lower limbs. The purpose of this study was to assess the role of the β-glucan in the induction of venous ulcers healing, especially through the identification and quantification of macrophages (CD68+ cells) and T-lymphocytes (CD3+ CD4+ cells and CD3+ CD8+ cells). A nonrandomized clinical trial with intragroup comparisons over time was performed. The study was approved by the Research Ethics Committee of the University Hospital Onofre Lopes (Nº147/09), Federal University of Rio Grande do Norte. Water-insoluble β-glucan was isolated from Saccharomyces cerevisiae and was dispersed in cream with crodabase CR2 to a final concentration of 3%; the cream was applied directly to the ulcer. A venous ulcer biopsy was taken before therapy initiation (day 0) and after 30 days of glucan treatment for detection and quantification of macrophages (CD68+ cells), and T-lymphocytes (CD3+ CD4+ cells, and CD3+ CD8+ cells) by immunohistochemistry. We followed 12 patients; nine were women and three were men. The median of the age was 59 years. Topical immunotherapy was applied to 13 venous ulcers. The duration of these ulcers was from 8 to 264 months, and the majority (61.5%) was recurrent wounds. Histomorphometric analysis showed a decrease of the macrophage number in all the cases after the treatment with glucan (p=0.001), and an increase of T-lymphocytes number (p=0.600). Among the cases showed an increase of CD3+, 7 (87.5%) also increased CD4+ marker, and CD8+ marker. Initially, the venous ulcers area varied from 8.38 to 66.89 cm2, and after the treatment with glucan the ulcers area varied from 4.84 to 58.02 cm2. We concluded that the glucan was able to modulate the immune response, accelerating the healing process of venous ulcers treated.
