SUMMARY:

Chronic Endometritis (CE) is characterized by the presence of persistent inflammation in
the inner lining of the uterus, the endometrium, and is associated with a variety of clinical
manifestations, including menstrual irregularities, pelvic pain, obstetric complications,
and infertility. Despite its clinical relevance, CE often presents significant diagnostic
challenges. The lack of specific symptoms and the overlap of clinical signs with other
gynecological conditions can hinder the early and accurate identification of the disease.
Furthermore, the available diagnostic methods can be costly, invasive, and/or have
limitations in terms of sensitivity and specificity. This difficulty in diagnosis highlights
the need for the development of an efficient and economically viable screening protocol
for the disease. In this context, this research proposes innovative approaches for the early
identification of CE and the development of an accessible, precise, and clinically relevant
protocol. To this end, a literature review was initially conducted, which comprises
Chapter 1 of this thesis, with the aim of presenting a panel that includes the main bacterial
genera associated with endometritis. The literature search was performed using
predetermined keywords in different databases, without restriction on language or year of
publication. The results of the review showed that the five most cited genera are:
Chlamydia spp., Ureaplasma spp., Streptococcus spp., Mycoplasma spp., and
Enterococcus spp., which are related to significant endometrial inflammatory alterations.
Subsequently, with the objective of comparing different methods for performing
endometrial biopsy in terms of their feasibility for diagnosing CE through
histopathological and immunohistochemical analyses for CD138, CD56, and PGP 9.5,
endometrial tissue samples were collected from patients over 18 years old in the surgical
hysteroscopy department at Maternity Hospital-School Januário Cicco (MEJC), NatalRN. This tissue was obtained through hysteroscopy, Cornier pipelle, and urethral catheter
attached to a syringe. No significant differences were identified between the
histopathological characterization and immunohistochemical staining when comparing
the three collection methods used. Therefore, the use of a urethral catheter attached to a
syringe is suggested as a new method for performing endometrial biopsy, being more
financially accessible, with a lower risk of post-procedure complications since it does not
require anesthesia, and as efficient in obtaining endometrial tissue as the other methods
mentioned, especially hysteroscopy, which is the most commonly used method in clinical
practice for this purpose. Taken together, these data show that the presence of the
mentioned microorganisms in the endometrium can compromise female reproductive
health, and that the use of a Nelaton-type urethral catheter adapted for endometrial biopsy
may emerge as a promising way to obtain endometrial tissue samples, either for
performing tissue microscopic analyses or for identifying microorganisms causing CE, as
well as identifying other uterine pathologies.
