SUMMARY:

Introduction: postoperative pain after cardiac surgery represents a relevant clinical problem, as it may compromise patient recovery and favor the occurrence of harmful physiological responses and complications when not properly managed. In this scenario, the adoption of effective strategies for pain control becomes indispensable. Among non-pharmacological alternatives, aromatherapy with essential oils is presented as a safe, low-cost intervention with potential for pain relief and relief of associated symptoms. Objective: to analyze the effectiveness of aromatherapy in pain relief in patients in the postoperative period of cardiac surgery. Methodology: this is a systematic review with meta-analysis, conducted according to the recommendations of the Cochrane Handbook for Systematic Reviews of Interventions and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement, registered on the International Prospective Register of Systematic Reviews (PROSPERO) platform under number CRD42024568532. The search was carried out in April 2025 by two researchers, independently and simultaneously, in the following databases: National Library of Medicine and National Institutes of Health (PubMed), Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Latin American and Caribbean Literature on Health Sciences (LILACS), Scopus, Web of Science, Cochrane Central Register of Controlled Trials (Library), and clinical trial databases (clinicaltrials.com). Randomized and quasi-randomized clinical trials on the use of aromatherapy for pain relief in adult patients in the postoperative period of cardiac surgery were included, with no restriction as to language or publication period. Data were extracted using a standardized form and submitted to descriptive analysis and meta-analysis in the Review Manager software (RevMan 5.2.3). Risk of bias was assessed using the Cochrane Risk of Bias 2 (RoB 2) tool, and the quality of evidence was assessed using the Grading of Recommendation Assessment, Development, and Evaluation (GRADE) approach. Ethical Aspects: as this is a study elaborated from secondary data, there was no need for submission for appraisal by a Research Ethics Committee. However, the precepts of Resolution No. 510 of 2016 were observed, especially with regard to the preservation of intellectual integrity and respect for the ideas, concepts, and definitions of the authors of the scientific literature used, in all stages of the study. Results: eight studies composed the final sample of this review, seven of which (87.5%) were conducted in Iran, totaling 599 participants, predominantly male, older adults and elderly individuals, and patients undergoing coronary artery bypass graft surgery. Different essential oils were used, with predominance of lavender, administered exclusively by inhalation, in short-duration interventions ranging from one to three days, and with doses between one and five drops. The reported adverse effects were intolerance to lavender inhalation, respiratory sensitivity, and nausea. Pain was assessed using the Visual Analog Scale (VAS) and the Visual Numerical Scale (VNS), vital signs by physiological parameters, and anxiety by the State-Trait Anxiety Inventory (STAI). In the meta-analysis based on the VNS, a statistically significant reduction in pain was observed in the intervention group compared to the control group (DM = -1.06; 95%CI [-1.52; -0.60]). For the outcomes anxiety reduction and vital signs control, no statistically significant differences were identified between the groups. Conclusion: aromatherapy is effective in relieving pain in the postoperative period of cardiac surgery when assessed by the VNS, with no consistent effects in reducing anxiety and in controlling vital signs. However, these findings should be interpreted with caution, considering the methodological heterogeneity, the risk of bias, and the low quality of the evidence observed in this review. Derivations of the Study: as derivations of this research, the publication of a protocol article stands out, as well as the planned submission of a scientific article with the final results of the dissertation. Social Impact: this research may contribute to the qualification of care provided to patients undergoing cardiac surgery, especially with regard to pain management in the postoperative period. In addition, by synthesizing and critically evaluating the available evidence on aromatherapy as a complementary intervention, the study may support evidence-based decision-making in clinical practice.
