SUMMARY:

Diagnostic cardiac catheterization is a highly complex invasive procedure
widely used in the investigation of coronary artery disease. The imminence of
this procedure is frequently associated with anticipatory anxiety. Nurses
working in hemodynamic units play a central role in promoting patient
well-being. Therefore, this study aims to evaluate the effect of immersive
virtual reality on anxiety management in patients during the pre-procedural
period of cardiac catheterization. The research will be conducted in two
stages. The first stage corresponds to a methodological study of content and
face validity carried out by expert judges. The second stage consists of a
prospective, controlled, single-blind randomized clinical trial, conducted in
accordance with the Consolidated Standards of Reporting Trials, in which
participants will be randomly allocated into an intervention group — receiving

immersive virtual reality through the Meta Quest 3 headset, displaying natural
scenery with the video's original audio for five minutes — and a control group,
which will receive exclusively the standard routine care of the hemodynamic
unit. Randomization will be performed electronically using the Research
Randomizer website. The study will be conducted at the Hemodynamics Unit
of the Onofre Lopes University Hospital, from February to June 2027.
Inclusion criteria comprise patients aged 18 years or older, electively
scheduled for diagnostic cardiac catheterization for the first time at the
institution, without recent use of sedatives or analgesics, and with preserved
cognitive, visual, and auditory function. Exclusion criteria include: diagnosis of
severe neurological or psychiatric disorders, use of anxiolytics or
antidepressants within the previous 24 hours, history of labyrinthitis, epilepsy,
or motion sickness, and refusal to use technological devices. The primary
outcome, anxiety level, will be measured using the Beck Anxiety Inventory,
administered at pre- and post-intervention time points by an assessor blinded
to group allocation. The project will be submitted to the Research Ethics
Committee of the Federal University of Rio Grande do Norte and registered in
the Brazilian Registry of Clinical Trials, with data collection initiated only after
all approvals are granted.
