SUMMARY:

Human milk is considered the gold standard for nourishing preterm infants. However, achieving nutritional adequacy through enteral nutrition therapy remains a challenge in Neonatal Intensive Care Units (NICUs), especially given the physiological limitations and low volumetric tolerance of these newborns. In light of this scenario, the use of pasteurized human milk (PHM), taking into account its energy variability, emerges as a strategy to optimize nutritional intake for preterm infants, which may promote the attainment of nutritional  intake recommendations established by guidelines for this population. Therefore, this study aimed to evaluate the effect of nutritional therapy using PHM in the form of colostrum followed by hypercaloric PHM on the adequacy of the enteral diet in preterm infants admitted to the NICU. This was a randomized, controlled, single-blind clinical trial in which 115 participants were randomized and allocated into two groups: the control group (n=57), which received expressed breast milk (EBM) and normocaloric PHM; and the intervention group (n=58), which received EBM and colostrum PHM until reaching a volume of 100 mL/kg/day, followed by hypercaloric PHM (≥70 kcal/100 mL). The intervention was maintained for at least seven days, until NICU discharge, or until a change in medical prescription entailed a shift in the planned feeding strategy for the group. The administered diet was monitored weekly (at 7 and 14 days of follow-up, and at NICU discharge) via medical records, collecting data regarding volume, frequency, diet type, and route of administration. Nutritional adequacy, the primary outcome of this study, was evaluated by meeting the recommendations for volume, energy, protein, and lipid intake through enteral nutrition therapy during the preterm infant's NICU stay. No significant differences were observed in the evolution of dietary intake between the groups. However, the intervention group presented a higher mean volume of EBM in the first weeks of follow-up and a higher proportion of preterm infants transitioning to oral feeding (36.1%). Further studies are needed to better determine the potential of this approach in optimizing enteral nutrition therapy for preterm infants.
