SUMMARY:

Odontogenic cysts constitute a complex and heterogeneous group of bone-destructive lesions whose clinical, biological, and histopathological diversity results not only from the intrinsic properties of the odontogenic epithelium but also from interactions with the lesional microenvironment, particularly macrophages and mediators such as TGF-β and MMP-9. In this context, this study aimed to analyze and compare the immunohistochemical expression of CD11c, CD163, TGF-β, and MMP-9 in dentigerous cysts and odontogenic keratocysts, as well as their associations with clinicopathological characteristics. A semiquantitative analysis of the distribution of immunolabeled mesenchymal cells was performed in 20 cases of each lesion. CD11c and CD163 expression was assessed in the cytoplasm of cells within the cystic capsule, whereas TGF-β and MMP-9 expression was evaluated in the nuclear and/or cytoplasmic compartments using scores ranging from 0 to 2. Associations among the parameters were analyzed using Fisher’s exact test, while differences between markers and cyst types were assessed using nonparametric tests, with a significance level of 5%. Regarding CD11c expression, most dentigerous cysts showed low immunoexpression, whereas odontogenic keratocysts predominantly exhibited high immunostaining; however, this association did not reach statistical significance (p=0.057). Odontogenic keratocysts showed significantly higher levels of CD163 and MMP-9 immunoexpression than dentigerous cysts (p=0.010 and p=0.011, respectively), whereas TGF-β expression was not associated with cyst type (p=0.723). Additionally, a significant association was observed between low CD163 and low TGF-β expression (p<0.001), as well as between high CD11c and high CD163 expression (p=0.001). The remaining associations among the markers were not statistically significant (p>0.05). Therefore, the findings indicate the differential involvement of CD163 and MMP-9 in the pathogenesis of odontogenic keratocysts, as well as a potential coordinated role of CD11c and CD163 and of CD163 and TGF-β, suggesting that interactions between immune cells and molecular mediators may contribute to the distinct biological behavior observed between these lesions.
