SUMMARY:

Gingival recession is a clinical condition associated with root exposure, cervical dentin hypersensitivity, and esthetic impairment, requiring predictable root coverage procedures. The subepithelial connective tissue graft (SCTG) remains the reference approach, although it is associated with morbidity at the palatal donor site. Albumin gel combined with platelet-rich fibrin (Alb-PRF) is an autologous biomaterial proposed as a less invasive alternative, combining the slowly resorbing denatured albumin fraction with the liquid PRF phase, rich in cells and growth factors, thereby avoiding a second surgical site. This randomized clinical trial evaluated, at 4 months, the efficacy of Alb-PRF compared with SCTG in the treatment of single RT1 gingival recessions. Nineteen participants were included, corresponding to 31 treated sites, allocated to coronally advanced flap using the Bruno technique associated with either Alb-PRF or SCTG. The treated tooth/site was the observational unit, and continuous quantitative outcomes were analyzed using linear mixed models, with treatment group as a fixed effect and participant as a random effect. The SCTG group showed a greater reduction in gingival recession height than the Alb-PRF group (1.58 mm vs 1.01 mm; mean difference = 0.57 mm; p = 0.001), a higher percentage of root coverage (73.98% vs 53.81%; mean difference = 20.17 percentage points; p > 0.005), and a higher frequency of complete root coverage (28.6% vs 0.0%; p > 0.005). SCTG also resulted in greater clinical gingival thickness gain (mean difference = 0.48 mm; p > 0.005) and more favorable behavior of the keratinized mucosa width (mean difference = 1.23 mm; p > 0.005). No significant between-group differences were observed for probing depth, clinical attachment level, bleeding on probing, categorical periodontal phenotype at 4 months, recipient-site pain, cervical dentin hypersensitivity, self-perceived esthetics, satisfaction, oral health-related quality of life, or Root Coverage Esthetic Score. In conclusion, at 4 months, SCTG outperformed Alb-PRF for the main clinical and dimensional outcomes. Alb-PRF demonstrated clinical applicability and low morbidity but did not show performance equivalent to SCTG in the evaluated protocol.
