SUMMARY:

Oral lichen planus (OLP) is a chronic immune-mediated inflammatory disease of the oral mucosa whose etiopathogenesis involves a T cell-mediated immune response. The infiltration of cytotoxic CD8+ T lymphocytes plays a central role in the apoptosis of basal keratinocytes, while pro-inflammatory cytokines such as interferon-gamma (IFN-γ) modulate the local immune response and contribute to the maintenance of the inflammatory process. Considering that the chronic inflammatory microenvironment may be associated with events related to the malignant transformation potential of OLP, the evaluation of these markers may broaden the understanding of mechanisms involved in both inflammatory progression and this biological potential of the disease. This study aims to analyze, through immunohistochemistry, the density of CD8+ T lymphocytes and the expression of IFN-γ in OLP samples, correlating the intensity and staining pattern with clinical and histopathological variables. This is a cross-sectional, retrospective, descriptive, and semi-quantitative study comprising 20 specimens of reticular oral lichen planus, 20 specimens of erosive oral lichen planus, and 10 fibrous hyperplasias as the control group. Clinical data regarding age, sex, and lesion location will be collected. Morphological analysis will be performed on hematoxylin and eosin-stained sections, as well as immunohistochemical analysis to investigate differences in IFN-γ expression and CD8+ T lymphocyte density among the groups. Data will be tabulated in Microsoft Excel (Microsoft Corporation, USA) and analyzed using SPSS for Windows (IBM, USA), version 20.0, applying the non-parametric Kruskal-Wallis and Mann-Whitney tests, adopting a significance level of 5% (p<0.05). Greater expression of these markers is expected to be identified in erosive OLP, contributing to the elucidation of the immunoinflammatory dynamics of the disease and the development of more targeted immunomodulatory therapeutic strategies.
