Introduction: Maxillary constriction and anterior open bite (AOB) are commonly intercepted with rapid maxillary expansion (RME) using an expander combined with a palatal crib. However, recent studies have observed a spontaneous increase in overbite among children with AOB treated with expanders without tongue-restraining devices. Objective: To evaluate the effects of RME with the Hyrax expander and the Hyrax expander combined with a palatal crib on the spontaneous closure of AOB in children during the mixed dentition stage. Material and methods: The sample will consist of 64 participants recruited from Clinic IV of the Department of Dentistry at the Federal University of Rio Grande do Norte (UFRN). Inclusion criteria will comprise children of both sexes, aged six to twelve years, diagnosed with facial pattern I or II, maxillary constriction, and AOB in the mixed dentition stage. Exclusion criteria will include participants with cleft lip and palate, craniofacial syndromes, absence of maxillary and/or mandibular permanent incisors, facial pattern III, and long face pattern. Selected patients will be randomly allocated into two study groups. The H group will undergo RME using a Hyrax expander, while the HPC group will undergo RME using a Hyrax expander combined with a palatal crib. Digital dental models will be obtained at two distinct time points: immediately pre-expansion (T1) and six months post-treatment, at appliance removal (T2). The primary outcomes variables will be overbite and the frequency of spontaneous AOB closure, while the secondary outcomes variables will include palatal depth; maxillary arch length; total arch perimeter, and anterior arch perimeter; maxillary intercanine, inter-deciduous molar or inter-premolars, and inter-first molar distances; inclination of maxillary and mandibular central and lateral incisors; and overjet. The data will be analyzed using the Kolmogorov-Smirnov test to assess the distribution pattern of the variables. If the data show a normal distribution, intergroup comparisons will be performed using t tests, and intragroup (T1-T2) comparisons will be analyzed using the paired t test. However, if the data do not meet normality assumptions, the Mann-Whitney test will be applied for intergroup comparisons, and the Wilcoxon test for intragroup comparisons. A significance level of 5% will be adopted. Expected results: No significant differences are expected between the groups regarding the frequency of spontaneous AOB closure and the increase in overbite after RME with the Hyrax expander and the Hyrax expander combined with a palatal crib.
