SUMMARY:

Introduction: Oral rehabilitation using dental implants has become one of the main therapeutic resources in contemporary Dentistry, aiming to restore masticatory function, phonetics, and aesthetics. The continuous development of prosthetic components has expanded clinical possibilities and increased patients’ aesthetic demands. Objective: To evaluate peri-implant aesthetics, patient satisfaction, quality of life, and clinical performance of single implant-supported crowns on one-piece (UP) and two-piece (DP) abutments in the esthetic zone of the maxilla. Materials and Methods: This was a longitudinal, double-blind, controlled clinical trial conducted at the Federal University of Rio Grande do Norte (UFRN), involving the placement of 30 implants sequentially allocated in the esthetic zone of the maxilla: 15 in the UP group and 15 in the DP group. Peri-implant aesthetics were assessed using the Pink Esthetic Score (PES); satisfaction, by the Visual Analog Scale (VAS); oral health-related quality of life (OHRQoL), by the OHIP-14 (Oral Health Impact Profile) and OIDP (Oral Impacts on Daily Performances); and clinical performance, by modified Ryge/CDA (California Dental Association) criteria. Follow-up periods included: baseline (T0), provisional crown installation (T1), and 1 (T2), 3 (T3), 6 (T4), 12 (T5), 24 (T6), and 36 months (T7) after cementation of the definitive crown. Treatments were considered equivalent if the 95% confidence interval (CI) for the mean difference in PES fell within -1.5 to +1.5 points. Statistical analysis included Mann-Whitney, SPANOVA, and Student’s t-test with Sidak post hoc (p<0.05). Ryge criteria were assessed qualitatively, and restoration survival was analyzed using Kaplan-Meier and Log-Rank tests. Results: Statistically significant differences between groups were found only for PES at T5. Both groups showed significant improvements in OHRQoL and satisfaction levels after the placement of definitive crowns (T2), compared to baseline (p<0.05). In the test group, one case presented with restoration failure (occlusal fracture) at 24 months, and another with marginal integrity failure (abutment mobility). Conclusion: Both one-piece and two-piece prosthetic abutments had a positive impact on peri-implant esthetics, patient satisfaction, and quality of life. Soft tissue conditioning and definitive crown placement appear to play a relevant role in optimizing these outcomes.
