SUMMARY:

Introduction: Burning mouth is characterized by a chronic discomfort sensation in the oral cavity, which may be idiopathic or associated with local or systemic factors. It represents a constant challenge in dental practice, as despite the availability of various therapeutic approaches, there is still no definitive consensus on the most effective one. Objective: To evaluate the effectiveness of Topical Capsaicin (TC), Photobiomodulation Therapy (PBMT), and Combined Therapy (PBMT + TC) in reducing burning mouth symptoms. Materials and Methods: This is a randomized clinical trial involving 22 patients with complaints of burning mouth, with or without systemic conditions. These patients were randomized into three treatment groups: TC (n = 7); PBMT (n = 7); and combined therapy (PBMT + TC) (n = 8). Each treatment lasted 30 days, followed by a 30-day follow-up period. Clinical and sociodemographic data were collected, and burning intensity was measured using the Visual Analogue Scale (VAS) at four time points: T0 – before treatment (baseline); T1 – 15 days after starting treatment; T2 – at the end of treatment (30 days after starting); T3 – 30 days after the end of treatment. Unstimulated salivary flow was measured at two time points (T0 and T2) using sialometry. Statistical analyses were performed using SPSS (v.20.0). Given the non-normal distribution and sample size, non-parametric tests were used: Fisher’s Exact, Friedman, Wilcoxon Signed-Rank, and Kruskal-Wallis, with a 5% significance level. Results: Most patients were female, over 50 years old, and postmenopausal. Systemic arterial hypertension was the most frequent systemic condition. Most patients were on medications, mainly antihypertensives. Burning and stinging sensations were the most reported symptoms. A statistically significant reduction (p < 0.0001) in symptom intensity (VAS) was observed over time in all three treatment groups, from the start of treatment (T0) to the end of follow-up (T3). The combined therapy group showed improvement already when comparing T2 (end of treatment) to T0. There was no statistically significant difference in VAS scores between groups or in delta comparisons. The clinical response rate was above 85% for all interventions, considered excellent. No statistically significant differences were found in salivary flow between the beginning and end of follow-up or in intra- or intergroup comparisons. Conclusion: All three therapies were similarly effective in reducing pain symptoms. TC stands out for being more economically accessible and having simpler usage guidelines, which may enhance treatment adherence and continuity. Despite the significant reduction in pain, no impact on salivary flow was observed, indicating no measurable changes in salivary gland secretory function
