SUMMARY:

Gingival recession is very common and can cause discomfort due to root exposure. Several techniques have been employed, demonstrating success in coverage rates. PRF (platelet-rich fibrin) is an autologous and accessible grafting option, promoting efficient initial healing, but with short duration. To overcome this limitation, Alb-PRF was developed — a combination with albumin gel that increases its stability for up to 6 months. This innovation presents superior biological properties and promising clinical potential. OBJECTIVE: To compare the quality of tissue repair in the treatment of single RT1 gingival recessions using Alb-PRF and subepithelial connective tissue graft (SCTG), aiming to elucidate whether Alb-PRF constitutes a substitute for SCTG and its effectiveness in tissue healing patterns. METHODOLOGY: This is a randomized controlled clinical trial in which systemically healthy patients, over 18 years of age, of both sexes, presenting at least one single gingival recession (RT1) will be randomly divided into two groups and surgically treated using Alb-PRF or SCTG. Patients will be longitudinally evaluated through clinical, imaging, and immunological analyses. Patient-centered parameters such as satisfaction level, postoperative pain, and gingival aesthetics will also be assessed over a period of up to 2 years. Data will be analyzed using Friedman, Wilcoxon, Mann-Whitney, Kruskal-Wallis, Chi-square, McNemar, ANOVA, and t-tests, with a pre-established significance level of 5%. EXPECTED RESULTS: Given that PRF acts as a natural stimulant, the greater stability of the Alb-PRF mixture is expected to extend its effectiveness over prolonged periods, elucidating its mechanism of action and expanding its capacity for tissue regeneration. This will contribute to understanding its role in the repair process and selecting the most appropriate surgical approach that promotes faster, more effective results with lower morbidity, while ensuring stability and durability.
