SUMMARY:

Introduction: Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease characterized by the degeneration of motor neurons. Nutritional interventions in ALS are essential and must be based on scientific evidence to positively influence the nutritional status, quality of life, and survival time. This systematic review aims to present a synthesis of evidence-based recommendations to support appropriate nutrition care for ALS patients.

Methods and analysis: The search was performed through PubMed/MEDLINE, Excerpta Medica Database (Embase), Scopus, SciELO, Web of Science, LILACS, ScienceDirect, Google Scholar, and manual search. We included clinical practice guidelines and systematic reviews in order to answer the research question: What are the evidence-based nutritional recommendations to maintain or restore the nutritional status of patients with ALS? This systematic review was developed by Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols (PRISMA-P). The AGREE II statement was used to assess the quality of the evidence and the strength of the recommendations.

Results: The electronic search strategy identified 461 reports. Of those, 11 were included in this review (7 guidelines and 4 systematic reviews). The overall AGREE-II scores for the included studies ranged between 3 and 7. The summary of nutritional recommendations was organized into five topics: 1) Dysphagia; 2) Energy; 3) Protein; 4) Supplementation; and 5) Percutaneous Endoscopic Gastrostomy (PEG). 

Conclusion: This review summarized relevant and updated nutritional recommendations to maintain or restore the nutritional status of ALS patients, contributing to their quality of life and survival time. These nutritional recommendations will help health professionals and caregivers in implementing and standardizing the nutrition care according to the evidenced-based practice in ALS. This review will be use as a foundation for the nutrition section of the first Brazilian guideline for multidisciplinary care in ALS.
