Introduction: Molar-Incisor Hypomineralization (MIH) is described as a developmental defect of the enamel, of systemic origin, which affects at least one permanent first molar, and may also be associated with permanent incisors. Clinically, it is characterized as demarcated enamel opacities that can vary in color (white-cream or yellow-brown). HMI is linked to a greater probability of developing caries lesions, hypersensitivity and esthetic impairment in anterior teeth, negatively impacting the quality of life of individuals with this condition. Objective: Investigate studies that assess difficulties in diagnosing MIH, estimate the occurrence and severity of molar-incisor hypomineralization in children and adolescents, and identify possible factors associated with the occurrence of MIH. Methodology: To answer the objectives of this research, two studies were carried out, a systematic review to answer the difficulties encountered in the diagnosis of MIH, in which observational studies (cross-sectional, case-control and cohort) involving children and adolescents were used. The search was performed in PubMed/ MEDLINE, Embase, Scopus, LILACS, Web of Science and Cochrane Central Register of Controlled Trials databases. And to answer the other objectives, a retrospective cross-sectional study was carried out in serial panels that used 230 medical records of patients aged between 06 and 12 years, undergoing orthodontic treatment at the Brazilian Association of Dentistry - Rio Grande do Norte section between the years of 2000 and 2021. The diagnostic criteria used were that each tooth should be analyzed according to the presence or absence of demarcated opacities, atypical restorations, post-rupturing fractures and molar extraction due to HMI. The collected data were typed and processed in the SPSS program, statistically treated using the chi-square test for a confidence level of 95%. Partial results: An association was observed between the occurrence of molar-incisor hypomineralization and the variables gender (male), age (06 to 10 years) and year in which the documentation was carried out (2012 to 2021). The occurrence of MIH throughout the study period was 26 cases among the 230 medical records evaluated, representing 11.3%, with a 95% CI of 7.21 to 15.39%.
