SUMMARY:

To reduce morbidity, cost, and clinical treatment time, single implant overdentures in the mandibular midline be an advantageous treatment alternative. This non-randomized controlled clinical trial aimed to evaluate the effect of installing an implant in the mandibular symphysis in totally edentulous individuals not adapted to conventional mandibular complete dentures (PT), compared to adapted users. We selected 20 bimaxillary edentulous patients who were rehabilitated with conventional complete dentures and waited 3 months to adapt to the new PT. The parameters that determined the adaptation were phonetics, chewing, and comfortable swallowing with the new dentures. After waiting for the adaptation time, the patients were divided into PTC Group - “adapted to mandibular PT” and PTI Group - “not adapted to mandibular PT”. Surgery was performed to install the implant, waiting 4 months for osseointegration and, after that time, the patients had their mandibular prosthesis converted into an overdenture. Before and after the intervention, satisfaction was evaluated based on a qualitative scale, masticatory performance (MP) using the sieve method with the chewing of artificial food, and the impact of oral health on quality of life using the OHIP- Edent-19; regarding clinical and radiographic parameters related to peri-implant health after rehabilitation with the implant-supported prosthesis, in 3-month follow-ups. A descriptive analysis was performed to characterize the study sample. The Kolmorogov-Smirnov normality test will be applied, whose analyzes will be obtained by means, standard deviations, medians, and frequencies. To obtain satisfaction data, the chi-square test will be applied. For analyzes between the study groups and the OHIP and PM measurements, in the face of non-normality of the data, the Mann-Whitney test will be used, while in the case of normality, the t test will be used. For pairing the groups, the paired t test or Wilcoxon test will be used, depending on whether the distribution is normal or not. For all statistical analyses, a significance level of 5% will be considered. The partial sample of the study consisted of 14 patients, of which 12 have mandibular overdentures and 2 are awaiting osseointegration. Most of the sample is composed of women (n=13, 92.9%), and the mean age is 65.07±4.51. Of the total number of participants, 64.3% (n=9) were not adapted to mandibular PT after 3 months of follow-up, and most had type III mandibular ridge height (n=8; 57.1%). 8.5mm (n=3; 21.4%), 10mm (n=3; 21.4%) and 11.5mm (n=8; 57.1%) implants were installed in these patients and the torque varied from 20 to 80 Ncm. It is expected to complete the surgical procedures of the 6 remaining patients by the end of February 2022, conversion of theprostheses between May and June, and carry out the follow-ups and data collection between July and September 2022.
