SUMMARY:

Despite relatively uncommon, benign and malignant tumors can develop in salivary glands. Salivary gland tumors (SGTs) have complex clinicopathological characteristics and exhibit a wide diversity of morphological aspects, biological behavior, and prognosis. Pleomorphic adenoma (PA) is the most common benign SGT. Among malignant SGTs, mucoepidermoid carcinoma (MEC) is generally referred to as the most frequent, followed by adenoid cystic carcinoma (ACC). In recent years, studies have provided strong evidence that tumor stem cells (TSCs) play important roles in tumorigenesis, including initiation, progression, metastasis, resistance to treatment and recurrences in various types of neoplasms. Nevertheless, in the scientific literature there are still few studies focusing on the biological significance of TSCs specifically in SGTs. The present study aims to describe the profile of TSCs in a series of cases of PA, ACC, and MEC, through immunohistochemical biomarkers CD44, OCT4, ALDH1, and SOX2, to better understand the different biological behavior and prognostic of SGTs, and also to elucidate whether there is a specific biomarker for identification of TSCs in the studied lesions. After collection of clinico-demographical data and morphological analysis, an immunohistochemical evaluation of CD44, OCT4, ALDH1, and SOX2 will be performed in 20 APs, 20 ACCs, and 20 MECs. Five microscopic fields (400x) in tumor parenchyma will be selected for analysis. In each field, a score based on the percentage of immunopositive cells (PP) and a score based on the intensity of expression (IE) will be attributed. Subsequently, the final score (FE) will be calculated using the formula: FE = PP + IE. Tumor stroma will also be evaluated, classified as negative or positive for each biomarker. The results will be analyzed statistically using the software Statistical Package for the Social Sciences version 20.0. A level of significance of 5% (p<0,05) will be adopted for all tests.
