SUMMARY:

Purpose: This non-randomized controlled clinical trial proposes to investigate  the incidence and risk factors to the adaptation of the conventional mandibular complete denture (CMCD). Materials and Methods: A total of 108 bimaxillary edentulous were rehabilitated with full dentures on a 6-month follow-up. Patients were divided into two groups: PTA (Patients adapted to CMCD; n = 67) and PTN (Patients non-adapted to CMCD; n = 41). The criteria that confirmed the patients’ adaptation were: chewing, phonetics, and swallowing comfortably with the dentures. Patients were evaluated in terms of sociodemographic aspects, patient-centered factors, and technical quality of complete dentures. A questionnaire was applied at the same time of installing the new dentures to obtain the sociodemographic and individual information of the patients. The technical quality of complete dentures (TQ) was evaluated clinically by technical factors related to the dentures. The risk factors were analyzed statistically by the Chi-square test or Fisher's exact test. Multivariate analysis was performed and the relative risk was adjusted by Poisson regression. The technical quality was evaluated by non-parametric Mann-Whitney tests in intergroup analyzes and Wilcoxon tests in intragroup analyzes, considering a 95% confidence interval. Results: The results revealed an incidence of 38% of individuals non-adapted to new mandibular complete dentures after 3 months. Non-adaptation to the mandibular complete denture was correlated with the low education level, the absence of past mandibular denture experience and with the higher use time of the lower CD (p<0.05). Dentures made by technicians, the presence of ulcerations after rehabilitation, and the height of the reduced posterior mandibular ridge demonstrated a significant correlation with non-adaptation to the complete denture (p <0.05). After 6 months, this incidence decreased to 14.1%, with the occurrence of ulcerative lesions being the only risk factor associated with the non-adaptation of CMCD. For intragroup assessments, TQ revealed a strong significant association for both groups (p <0.05). Conclusion: It concluded that after 3 months, the low education level, the absence of previous experience with CMCD, higher use time of the lower CD, the use of CD made by a non-dentist, registration of traumatic ulcers in the mandibular arch, and the presence of lower mandibular ridges interfere negatively in the adaptation to new lower dentures, constituting risk factors for non-use CMCD. However, after 6 months of wearing the prostheses, only the occurrence of ulcerative lesions is a risk factor for non-adaptation to mandibular prostheses.
