SUMMARY:

The fundamental principles of the Unified Health System (SUS), such as universality, comprehensiveness, and equality in access to healthcare, reflect a commitment to the comprehensive provision of services. However, the discussion about access goes beyond availability, encompassing health impact and care quality. The challenge of decentralization and regionalization of the healthcare system in Brazil is notable due to regional disparities and the complexity involving multiple stakeholders. Although decentralization provides greater autonomy to municipalities, it reveals challenges, especially in specialized care, due to the resolution of Primary Health Care and the fragmented organization of healthcare services. The National Regulation Policy of SUS seeks to operationalize regulatory functions, highlighting Referral Protocols as tools that guide clinical and management decisions, aiming to improve the effectiveness and efficiency of the healthcare system. In this context, this dissertation aims to develop a personalized referral protocol for patients with orthopedic conditions in a school clinic. Therefore, the present research will be a descriptive case study with a qualitative approach. Data collection will be carried out at the Multicampi School Clinic of Medical Sciences in the city of Caicó - RN. The approach will include the analysis of the efficiency of care, considering variables such as waiting time, quality of services provided, and effective utilization of resources available at the clinic. Additionally, the referral flow of patients from Basic Health Units to orthopedic specialty services will be analyzed. Data will be collected through direct observations, documentary records, in order to inform the creation of a protocol that optimizes patient flow and contributes to efficient resource management at the school clinic. Finally, the 5W2H tool will be applied for the formulation of a referral protocol within the school clinic, consisting of an exploratory and evaluative methodology. Results showed that despite the strengths of the current system, challenges related to the referral of orthopedic patients were identified, such as referrals without proper justification or reason, the lack of Rheumatology professionals in the municipality, and referrals solely for medical reports. The creation of a clear flowchart and standardized referral form are important initiatives in this process, facilitating communication among different stakeholders and ensuring that all necessary information for adequate patient care is provided. These tools not only optimize the referral process but also contribute to more effective and personalized care, aligned with the specific needs of each patient. Furthermore, the Orthopedic Referral Protocol presents a systematic approach to different orthopedic conditions, providing clear criteria to guide healthcare professionals in decision-making regarding the need for referral. Its organization by specific diseases and conditions facilitates the identification of situations requiring referral and promotes a more precise and individualized approach for each patient. In this context, it is concluded that it is essential to optimize referral processes, adopting more careful approaches in assessing patient needs and promoting better coordination among available healthcare services.
