SUMMARY:

Colorectal cancer presents high biological heterogeneity and distinct clinical behaviors that challenge
conventional staging. Given the need to improve prognostic stratification, this study evaluated the
clinicopathological characteristics, survival, and tumor aggressiveness of colorectal cancer in patients
treated at Liga Norte Riograndense contra o Câncer, em Natal, Rio Grande do Norte, Brazil. A retrospective
longitudinal study was conducted with a cohort of 112 patients diagnosed between 2018 and 2023 at the
Pathology Laboratory of Liga Contra o Câncer in Natal-RN. In the longitudinal study, clinicopathological
characteristics and gene expression by RT-qPCR of seven tumor markers were analyzed: exportin-1

(XPO1), programmed death-ligand 1 (PD-L1), Testis-Expressed Protein 46 (TEX46), cyclooxygenase-2 (COX-
2), melanoma-associated antigen (MAGE), cancer-associated gene (CAGE), and carcinoembryonic antigen

(CEA), all associated with tumor progression, in 21 patients with colorectal cancer, predominantly at TNM
stages III and IV. The retrospective study comprised 91 patients who were evaluated for
clinicopathological characteristics, including overall survival using the Kaplan-Meier method and Cox
regression models. Among the 112 patients, the mean age at diagnosis was 63.3 years, with a
predominance of females (57.1%), and 61.6% of cases were classified as stages III and IV. Mortality was
significantly associated with lymphatic invasion (OR = 5.51; p < 0.001) and distant metastasis (OR = 5.11; p
< 0.001). For advanced staging, metastasis showed the strongest association (OR = 13.33; p < 0.001),
followed by lymphatic invasion (OR = 5.14; p < 0.001) and perineural invasion (OR = 4.95; p = 0.007).
Median overall survival was 27.1 months. Among the 21 tumors analyzed by qPCR, a strong co-expression
signature among the markers was identified, particularly the correlations between XPO1 and TEX46 (ρ =
0.942), PD-L1 and TEX46 (ρ = 0.921), and COX-2 and TEX46 (ρ = 0.956). In conclusion, adverse clinical
outcomes were primarily associated with mechanisms of local and systemic tumor dissemination, whereas
the coordinated expression of XPO1 and PD-L1 suggests shared biological pathways associated with
colorectal cancer aggressiveness
